An opportunity has arisen for enthusiastic,motivated, hardworking individuals to join the Assertive Outreach (AO) Service. A RMN or Registered Mental Health Nurse qualification is essential.

The AO Service is a community based tertiary service who take referrals from CMHTs, Forensic Service, PIER and in-patient areas. We operate a team approach providing comprehensive care packages for people withsevere and enduring mental health problems residing in Leicester,Leicestershire and Rutland.

Candidates will need to demonstrate a commitment toproviding comprehensive care packages for people with enduring mental healthproblems within their own homes. Excellent assessment, risk management,organisational and communication skills are essential along with experience anda good working knowledge of the Care Programme Approach, Community mentalhealth legislation and Hospital admission / discharge process.

We offer a hybrid model of base and homeworking, a robust induction, supervision and ongoing training. There are excellent opportunities with adult mental healthcommunity for promotion and career progression.

Main duties of the job

The successful candidate will assess, plan, deliver and evaluate care for service users who are referred to the Assertive Outreach Service, including the assessment and management of risk,working within a psychosocial / family centred framework when assessing and planning individual packages of care.

The post holder will be responsible for organising and planning your work as an autonomous practitioner managing your time effectively and liaising closely with other agencies and disciplines. Undertaking the role of CPA co-ordinator for a case load of service users with complex mental health problems who are difficult to engage and require intensive packages of care.

To develop therapeutic relationships with service users in order to provide support and assistance and toadminister and monitor the effects and adverse effects of prescribed medication and liaise with medical staff regarding any necessary adjustments in medication. To offer advice / information to clients and carers in regard to prescribed medications.

Registered Mental Health Nurse level 1 with NMC registration.

Evidence of continuing post registration/graduate professional development/training relevant to the service area eg PSI/CBT.

Knowledge and Skills

Essential

Demonstrate a comprehensive knowledge of the AO model. Relevant knowledge in mental health clinical practice, CPA process, MHA 1983 etc. Knowledge of Clinical Governance, Research, Audit processes and Standard Setting. Understanding of Community Care Legislation. Ability to work independently Assessment, Care Coordination and evaluation skills. Ability to assess and manage clinical risk. Ability to prioritise / organise workload (own and team). Ability assesses people with complex mental health problems and collaboratively develops and review care plans (including risk management plans). Working knowledge and understanding of safeguarding vulnerable adults & children. Computer/IT skills
