Band 5 Occupational Therapist - Foundry PCN Job overview

To provide a direct specialist mental health Occupational therapy and interventions to adult and older adult service users with severe mental health needs, (including complex needs associated with personality disorders/emotional sensitivity) within the Primary and Community Mental Health programme, their families and carers.

To develop and maintain effective working relations within the multidisciplinary team representing your professional background as an Occupational Therapist. Each month there is dedicated professional development time delivered by experienced clinical leads. You will be managed and supervised by an experienced Occupational Therapist.

To provide specialist psychosocial assessment and advice and consultation on clients' mental health and wellbeing with colleagues from Primary care settings and within your multidisciplinary team. You will to work autonomously within professional/voluntary sector colleagues. You will work to the services' policies and procedures which is delivered in partnership with Primary Care Sheffield and Sheffield Mind.

Provide a range of psychosocial treatments and interventions working within their scope of practice, Carry out risk assessments in relation to psychosocial distress and to ensure that risk formulations are integrated with interventions understanding contingency management and the use of risk indicators in mitigating against crises Develop collaborative formulations with patients so as to sense-check understandings and influence delivery of evidenced-based individualised interventions. Apply mental health interventions that are consistent with self-management strategies and action plans for people with complex and chronic needs. This will be delived through the 'lens' of your profession and within an agree number of sessions. To understand of the needs and assets of individuals and families from a wide range of racial, cultural, sexual, religious and social backgrounds. To join with service users and colleagues, co-produce innovative ways of communicating and working across language and cultural differences. Work as part of a multidisciplinary community team or in specialised clinical settings and liaise with relevant external agencies to facilitate and enable recovery Plan and measure outcomes for patients using established national tools.

Job responsibilities

The post holder will provide mental health interventions aligned to their professional registration. This will be delivered within the Primary and Community Mental Health Service. In particular, those with serious mental illness and complex mental health problems, focusing on strength and assets to help people with what matters most to them.

The characteristics of the core population receiving direct services are: people with complex psychological and emotional needs who may have experienced adverse life events. Those with serious mental illness, for example complex trauma personality disorder, psychosis, schizophrenia and complex depression and anxiety (the list is not exhaustive).

The post holder will work with people who use our services to develop a shared understanding of the complex issues the person is facing, how they cope and the impact this has on their mental, physical health and well-being and those around them. They will explore social, psychological, therapies and other potential options which may improve health outcomes and help them achieve agreed goals. The post holder will provide treatments and will not be expected to be a care coordinator or assessment only. The post holder will have dedicated time with patients for up to 15 sessions using evidence based OT interventions and tools. Sessions are typically 45mins. Adaptations can be made to adjust for additional needs such as neuro diversity, learning difficulty, working with translators etc.

Service users seen by the post holder will include those who have experienced high levels of distress and complex mental health difficulties. Service users may present with a high level of risk to both themselves and sometimes to other people but this will be managed as part of a team in the Foundry PCN. Assessments and interventions will be delivered in a range of settings, including community, VCSE as well as clinics and in person, by telephone or digital platform.

A key aim of the service is to help people build resilience and feel more connected to their community, and to instil a sense of belonging.

Knowledge and Skills

Essential

Ability to assess complex health and well being needs and develop appropriate OT interventions Application of OT process with designated client group Ability to keep accurate and concise records Word processing and IT skills Basic knowledge of the principles of clinical governance

Knowledge of a range of mental health conditions and relevant mental health legislation Group work skills Vocational Rehabilitation skills

Training and Qualifications

Essential

Degree/Diploma in Occupational Therapy Registration with Health Care Professions Council

Desirable

Placement Educator Accreditation L'APPLE or willingness to undertake within first year

Experience

Essential

Previous experience in adult mental health environment Participation in and documentation of continuing professional development

Desirable

Experience of supporting people with vocational/educational goals. Experience of working in the community
