Job Requirements:

You will be a qualified Nurse or due to qualify RMN/RNLD. You will have a real passion for leading your teams in delivering safe care that you can be proud of and you will be comfortable making clinical decisions with the support of your Senior Nurses and Nurse Leaders. Job responsibilities

Main Role Purpose

To work according to the NMCCode of Professional Conduct and other professional and legislative guidelinesas a named nurse for a number of patients, taking responsibility for:

Protecting, supporting and improving the health ofindividual patients

Assessment of holistic care needs for patients.

Development, implementation and evaluation of careand treatment in collaboration with the individual patient.

Undertake active participation in effectivedischarge planning

Toensure the optimum quality of care for patients at all times.

Tosupport the Senior Staff Nurses, Managers and senior clinical staff in allaspects of ensuring high standards of Care delivery and compliance.

Towork proactively within the wider multidisciplinary team, fostering team workand ensuring the holistic assessment and treatment needs of the patients arebeing met.

Toassist in and support staff to achieve high quality care through innovative andevidence based practice.

Toact as an ambassador for the service and organisation through all interactions upholdingthe reputation and professionalism of the Team.

Main Duties/Responsibilities

Act as aNamed Nurse for identified patient(s)-ensuring all aspects of Care Planning areimplemented and reviewed according to relevant protocols.

Promoteand engage patients actively in involvement in their care.

Ensureall Care plans have risk assessments that are relevant, clearly identified andrisk management plans are clear and consistent.

Ensurethat all standards of ward procedure are implemented to the highest standard.

Ensurepolicies for the safe storage, custody and safe administration of medicines arecarried out to the highest standard.

Assistthe management and multidisciplinary teams in ensuring that all Clinical documentation/procedurescomply with all applicable legislation.

Undertaketo provide clear concise, accurate and objective documentation in the form of:

o Reports

oDailyrecording entries

oMeasurementand result recording

oMedicineadministration records

oSecurityforms

oFinancialrecords

oOtherdocumentation related to job role

Attend and take part in Ward Rounds, CPA , MHRT as required.

Ensurecorrect procedures take place for the Admission and Discharge of Patients.

Assistin directing, supporting and training Junior staff in the management of workflows.

The rolecan be physically demanding and there is a requirement for all ward-based staffto be able to undertake physical intervention training; and to be able toutilise these skills post training.

Assistin ensuring all relevant KPIs and Outcome measures are met, relevant to the Ward

Maximisethe use of resources and reduce waste/usage where possible

Human Resources

Maintaina professional position at all times.

Maintaina personal professional portfolio and ensure that you implement evidence based clinicalpractice.

Undertakeand promote active engagement in clinical supervision, carryout clinicalsupervision for Junior Staff as required.

Take anactive stance in the professional development and support of Junior staff

EnsureSupport staff are only asked to perform tasks that are within their area ofresponsibility and levels of competence.

Health and Safety

Attend allHealth and Safety updates and training required as part of hospital statutorytraining.

Assistin the maintenance of physical security and safety through regular monitoringand checks.

Activelyundertake responsibility for the Health, Safety and Welfare of him/herself andother employees in accordance with legislation and specific Policies andProcedures.

General
