An exciting opportunity has arisen for an experience nurse to join the Myeloid multidisciplinary team based at the Churchill Hospital in Oxford. The Myeloid MDT provides care for local patients and tertiary level care for the Thames Valley region. The team includes a Band specialist nurse and clinical and academic medical staff . The post holder will liaise with the inpatient, outpatient, ambulatory, BMT and research teams, primary care and regional hospitals. Patient-centred care is our priority, so our patients receive high class, personalised treatment.

Patients looked after by the team include those with acute leukaemia (AML and ALL) together with a range of myeloproliferative disorders including CML, myelofibrosis, essential thrombocythaemia and polycythaemia. The treatment of these disorders is going through an era of rapid progress and development; this gives the post-holder the opportunity to support and develop a world-leading clinical service for this group of patients.

Main duties of the job Use their expert knowledge and clinical judgement to decide whether to refer patients for investigations and discuss potential diagnoses with members of the wider MDT. Decide on and carry out treatment, which may include advice on the prescribing of medicines, or referring patients to an appropriate specialist. Assess individuals, families and populations holistically using a range of different methods, using physical assessment and advanced history taking skills and other specialist health needs assessments. Ensure the provision of continuity of care. Assess and evaluate, with patients and /or carers, the effectiveness of the treatment and care provided and make changes as needed. Work autonomously within a wider health care team and as a lone practitioner. Work across professional, organisational and system boundaries and proactively develop and sustain new partnerships and networks to influence and improve health, outcomes and healthcare delivery systems.
