Job summary

Hywel Dda University Health Board presents an exciting opportunity for two motivated and experienced Physiotherapists to form a rotation between our NHS & Public Health Award Winning "Home First Team" (Intermediate Care Services) and Front of House Transfer Of Care Advise and Liaison Service (TOCALS) in Prince Philip Hospital, Llanelli, Carmarthenshire.

Main duties of the job

The post holders will input into our home based and bed based intermediate care services here in Carmarthenshire.

As a versatile postgraduate, you will need to have a strong clinical grounding in older people rehabilitation, expertise in frailty syndrome, neurological conditions, cardio respiratory conditions, MSK problems and a good understanding of current issues affecting acute, intermediate care and community services.

Successful applicants will require an excellent ability to work autonomously, managing and prioritising a diverse and unpredictable caseload, with the ability to adapt to a rapidly changing and pressurised environment alongside members of the MDT. Post holders will be expected to support and play a role in supervising junior Physiotherapy staff, Physiotherapy Technicians, Rehabilitation Assistants/Frailty Support Workers, and students.

You will be required to participate in the respiratory on-call service and the potential future provision of a flexible 7-day service, based on service need. This will be on a rota basis.

Job responsibilities

Home First Team: We are an innovative team consisting of Health, Social and Third Sector services based in the community, striving to deliver the right support, at the right time in the right place. Our approach is to deliver, holistic person-centred care while supporting patients, when possible, to stay or return home in a safe manner.

TOCALS: The MDT works closely together to provide rapid assessment at the Front Door (AMAU and MIU) with the aim of reducing length of stay for the frail and unwell population. Community resources are utilised, alongside the ability to outreach as a service, ensuring safe and timely discharge.

Both teams work seamlessly together to implement the Home First approach in Carmarthenshire aiming to provide holistic, person centred care to patients as they transition between acute and community sectors. We work together to provide early intervention, safe and timely discharge with rehabilitation and assessment in the community to mitigate loss in function, deconditioning, nutritional deficit and deterioration in mental health to increase peoples time spent at home.

The post holder will be required to participate in the provision of a flexible 7-day service, based on service need and rota basis. They will have the skills required to participate in the respiratory On-call service.

The ability to travel to location in the community is required to support the out-reach services provided by TOCALS. Cross site cover may be required (Glangwili General Hospital).
