Job summary

AtBridgewater Community Healthcare we have an exciting opportunity available forthe right candidate to join our District Nursing service in Warrington. We arecurrently looking for a District Nurse Coordinator to lead our District NurseOut of Hours service.

Applicants willneed to demonstrate excellent communication skills, a sound ability to prioritiseworkload, time manage, lead on developing plans of care as well as support/mentorjunior staff members within in the service. Thesuccessful candidate will be expected to assist the OperationalManager in maintaining operational, clinical, professional, organisational, andnational objectives. The post holder will have the autonomy to actindependently and is responsible for leadership, maintaining and improving allaspects of clinical governance, evidence based practice and quality assurance.

Main duties of the job

The post holder will work as a senior member of the District Nursing Team collaborating and support the provision of clinical leadership within the team. A key feature of the role is to.

Support and deliver a high quality, safe and effective out of hours service provision Warrington residents.

Perform comprehensive assessment of highly complex patient nursing needs.

Ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care.

Work collaboratively with the Primary Care teams other professional and agencies to ensure patients needs are met.

Participate in the health needs assessment of practice populations to identify areas of unmet need.

Demonstrate the ability to work without direct supervision and to solve problems.

Provide advice, guidance, and support to District Nursing staff on all clinical and professional issues, vulnerable adults, legal and ethical issues in nursing.

Be responsible for the equitable distribution of workloads across teams. Manage the District Nursing Services through the development and supervision of staff in order to maintain an efficient service and ensure District Nursing teams and caseloads are managed effectively.

Lead, implement and monitor, within the locality, developments in the District Nursing service that are in line with local and national directives.

Job responsibilities

Clinical:

To perform comprehensive assessment of highly complex patient nursing needs. Plan, assess, implement, and evaluate care delivery according to changing health needs using prescribing skills. To ensure practice is evidence based through undertaking audit and critically appraising literature to achieve a high standard of care. To actively contribute to multidisciplinary meeting as appropriate. To establish and maintain effective communication with individual groups and communities in complex and potentially challenging situations. To involve patients and carers in the planning and delivery of care. To competently perform a range of clinical skills related to the post. To maintain accurate records in accordance with NMC standards. To identify skills and knowledge to address self and individuals training needs by undertaking Personal development plans. To demonstrate the ability to work without direct supervision and to solve problems. Provide advice, guidance, and support to District Nursing staff on all clinical and professional issues, vulnerable adults, legal and ethical issues in nursing. Act as an agent for highlighting and disseminating best practice in nursing. Supervise and monitor professional standards as laid down by the Nursing & Midwifery Council (NMC). Ensure record keeping standards are met in accordance with the NMC Guidelines for Records and Record Keeping.

Managerial/Leadership:

To manage time to ensure that 60% of time is spent on managerial duties and 40% clinical. To be responsible for the equitable distribution of workloads across team. Manage the District Nursing Services through the development and supervision of staff in order to maintain an efficient service and ensure District Nursing teams and caseloads are managed effectively. Lead, implement and monitor, within the locality, developments in the District Nursing service that are in line with local and national directives. Lead and chair working groups specific to the District Nursing service. Take designated lead responsibility for all of the following at locality level, education, audit, Out of Hours, treatment room services, phlebotomy services and Clinical Risk Management. Represent the service at multi-agency and multi-disciplinary forums and facilitate/chair as Monitor service delivery and highlight resources required to respond to the changing needs of the service. Ensure that appropriate information is available and evidenced to the Operational Manager to inform the strategic development plan. Manage all personnel issues including complex personnel issues within own area of responsibility, capability, conduct, behaviour, fraud, managing attendance. Manage flexible working and innovation appropriate to practice. Lead the recruitment process, interview, and select staff and chair interview panels. Actively participate in retention and professional development of staff. Ensure induction, preceptorship and mentorship programmes are implemented within area of responsibility. Including development of competency packs. Develop and implement Standard Operational Procedures (SOP). Implement Trust Policies, Procedures and ensure staff adherence through education and audit. Investigate accidents, formal and informal complaints, and untoward incidents in accordance with Trust policy and produce, implement and monitor action plans, as appropriate. Reporting to the CCG panel as required. Undertake investigation on pro-active management of individual and team performance issues within the locality, including grievance and harassment, poor performance, conduct and ill-health, seeking HR and line management support as necessary. Ensure staff compliance with recording and monitor activity patterns in order to inform decision making re service delivery, allocation of resources, skill-mix, etc. To take the lead role in maintaining and improving all aspects of clinical governance, evidence based practice and quality assurance. Manage and maintain ESR data base including all staff activities. To ensure that weekend working is coordinated across the individual division. Monitor locality budget and ensure resources are utilised effectively.

Clinical Governance:

To lead the clinical governance agenda by benchmarking best practice identifying any inequalities in service delivery Have in depth knowledge of the legal, ethical, moral and accountability issues related to district nursing Assess clinical and non-clinical risk that may affect patients and care providers working within the trusts risk management policy To support the public health agenda To maintain a professional portfolio that demonstrates evidence of on-going professional development To identify any limitations gaps in professional and clinical knowledge

Teaching and Learning:

To ensure own continued professional development and partake in clinical supervision. To maintain a personal, professional portfolio by collecting evidence of personal development through reflective practice. To undertake mandatory training that is specific for the post. To facilitate the placement of pre and post registration students. To support the development of the nursing teams by teaching and assessing all members of the teams including nursing auxiliaries Create within teams support mechanisms for continuing professional development. To teach patients/carers and other professionals to support patients to maintain their optimum level of independence. Develop and manage training needs analysis for Out of hours staff. To support the teaching and the assessing of the Specialist practitioner students Ensure alignment appropriate numbers of community practice teachers (CPTs) and ensure CPTs have support with their responsibilities to Specialist Practitioner students and Higher Educational Institution (HEIs). Facilitate effective learning within community practice and nurture reflective practice. Relate national policies to the education of learners and staff working in community nursing practice. Deliver in-service training as required via various teaching methods. Acknowledge the public health role and actively seek out opportunities for improvement and protection of the health and well-being of the wider community. Manage and coordinate all staff for mandatory group clinical supervision in line with NMC requirements. This includes monitoring of compliance. Person Specification
