Job overview
An exciting opportunity has arisen for a young adult ( 18-25) Senior Mental Health Practitioner. This is a newly developed post, created to enhance existing pathways into adult mental health services for young people. The post holder will work alongside the adult Mental Health and Wellness Teams providing locality based care within the London Borough of Redbridge.
The post holder will require vision and drive to support and implement new ways of working to shape transition care pathways in supporting young adults in preparing for adulthood. The newly created role will be pivotal in shaping the care pathway from children services to adult services, to ensure high quality, timely, accessible , evidence based service and improving service user outcomes.
The post holder is required to be able to take leadership responsibility of the pathway.
Starting with NELFT
NELFT place a great deal of importance on new starters being properly welcomed and inducted into the Trust. All new starters will join the Trust on the first Monday of each month and will undertake a comprehensive induction of up to two weeks which will include mandatory training, systems training and the allocation of equipment. As part of the process new starters will have the opportunity to also meet the executive team, senior managers and attend a number of drop-in sessions focusing on engagement, health and wellbeing and key processes. The induction will be held at our head office in Rainham, Essex.
Main duties of the job
They will be a qualified practitioner and fully registered with the appropriate professional or regulatory body ( HCPC, NMC or Social Work England).
The post-holder will provide clinical leadership to the Young People and Adult Mental Health Professionals working in the care pathway across children and adult mental health services from aged 18 to 25 years.
They will be responsible for assessment, intervention and systematic outcome measurement in the care pathway for this service user group.
They will ensure that a high quality, timely, accessible and evidence-based service is available to all service users who need it and who use the care pathway transitioning from children services to adult services
The post-holder will establish and maintain excellent working relationships with wider mental health and wellbeing services and ensure adherence to evidence-based interventions by staff.
The post-holder will ensure that there are no delays in young people accessing timely adult services, either as new referrals or for those transitions from CAMHS.
The post holder will have good working relationship with all the three mental health and wellness team in Redbridge including EIP to ensure patient recovery journey and transitioning is smooth and safe.
The post holder will have good working relationship with the Redbridge MH&WT referral coordinator and attending referral meetings when they can.

High-Cost Area Supplement
This post also attracts payment for High-Cost Area Supplement of 15% of the basic salary (minimum payment of £4,551 and a maximum payment of £5,735) pro rata for part time staff.

Detailed job description and main responsibilities
To provide an efficient, effective, comprehensive and specialist care pathway for young people aged 18-25 with emotional and mental health problems, their carers/parents and families, providing and ensuring the provision of multi-disciplinary assessment and intervention. To undertake a range of generic clinical work, urgent assessment work, parenting support and specialist cases. To provide consultation on care pathway cases to other multi-disciplinary staff within CAMHS and Adult Mental Health. To contribute to audit and research. To actively engage in treatment and outcome monitoring. To work in professional practice, making use of supervision, within the overall framework for mental health in line with Trust’s policies and procedures To use where appropriate, an assertive outreach approach to minimise the risk of YP/YA falling between services To support client within the pathway with crisis support though ICAH/HTT and inpatient ward as appropriate.
