Job summary

Boston Primary Care Network is looking for an enthusiasticself-motivated person to be the important link between a GP Surgery and localcare homes. This is an administrativeposition and does not have any clinical care.You will be the main point of contact for care homes, checking in withthe homes on a regular basis, accessing support for residents as needed andmaintaining clinical records.

You will need to have excellent communication skills, bothspoken and written. We are also lookingfor people who can build positive relationships with practices, care homes,wider PCN team and other people/organisations linked to patient care.

Main duties of the job

To play a role in the delivery ofhigh-quality primary health care services To establish and maintain effectiveliaison with stakeholders including health, voluntary, social and educationresources, attending relevant meetings as necessary Refer people and/or introduce them toappropriate organisations in Boston and nationally (where appropriate) voluntary, statutory (local authority) and local NHS organisations. Support the PCN Manager in providingKPI reports for submission as requested. Be responsible for the organisation,planning and of own workload to meet set deadlines. Following PCN and practice policiesand procedures as appropriate. Undertaking Weekly care home checkins Responding to ad hoc requests fromcare homes Organising relevant practice meetingsand actions Building positive relationships withcolleagues and internal/external stakeholders Supporting improvement projectsrelated to care homes

Job responsibilities

Care HomeCoordinators provide a vital link between individual GP practices and localcare homes. The role is pivotal inensuring all people receive the best possible coordinated care and access toservices. The Care Coordinators rolewill support GP practice colleagues and the Multi-Disciplinary Team incoordinating all key activities, including access to services, MDT meetings,advice, and information, and ensuring health and care planning is timely,efficient, and person-centred.

Each Network Practicehas a care home coordinator and while most of the time work is within anallocated GP practice and care homes, working as part of a wider team isimportant, to provide cover when colleagues are not at work and to help sharethe workload.

Essential

Computer literate with the ability to use Microsoft Office Programs such as Word and Excel competently with some experience of the other programs of the Microsoft Office package Understand the aims of current healthcare policy within the PCN Able to analyse and interpret data Has attention to detail, able to work accurately, identifying errors quickly and easily Has a planned and organised approach with an ability to prioritise their own workload to meet strict deadlines Able to think analytically; anticipating obstacles and thinking ahead; using analytical techniques to draw logical solutions to problems Excellent communication skills, verbal and written, with the ability to adjust communication style and content to suit the audience An excellent understanding of data protection and confidentiality issues Able to arrange suitable meetings with multiple individuals with often conflicting priorities

Working knowledge of SystmOne

Qualifications

Essential

GCSE (A* - C) or equivalent in Maths and English Evidence of continuing professional development Experience in care coordination or clinical administration

Experience in a health or social care environment
