Job summary

Responsiblefor managing the implementation and delivery of the PCN DES Servicerequirements across the PCN, supporting the achievement of the Impact &Investment Fund (IIF) targets across PCN practices, as well as operationalmanagement of other identified projects and services the PCN are required todeliver annually.

The rolewill also involve liaising with PCN Practices and other stakeholders to promoteengagement and understanding of the PCN specifications and requirements forimplementation.

The postholder will work with the PCN Management Team (including the PCN ClinicalDirector, and PCN Business Manager) to progress new and innovative servicedevelopments responding to emerging knowledge and techniques, reporting to theClinical Director.

Main duties of the job

To provide oversight and coordinate delivery of the PCN DESspecifications and IIF targets as relevant. To liaise with PCN leads inrelevant areas.

Ensure all relevant national strategies, policies, targets, and frameworksare fully implemented as required.

Ensure a whole system personalisedcare approach across the PCN that enables a variety of services across thehealth, social care, public health, and community spectrum to be integratedaround the individual in order to deliver better outcomes and experiences.

Deliver on the PCN guidancedocuments associated with the PCN DES (ie enhanced health in care homes,personalised care)

To drive issues through to resolution, acting as a point of expertise

Work with the PCN Clinical Director, PCN Business Manager and board forstrategic and operational planning processes to support PCN objectives asabove

To implement network-based projects and services,alongside PCN management team.

About us

West Northumberland Primary care Network comprises of 10 core practices and 2 further practices for whom we supply services. Most practices are rurally located and the PCN covers a large area.

The role is 16 hours a week (Tuesday and Thursday), based in Ponteland at White Medical Group.

Responsibilities include:

Keep abreastof current affairs and identify potential threats and opportunities.

Contribute toPCN strategy.

Monitor andevaluate performance of the PCN service requirements.

Develop andmaintain effective communication within the PCN and the PCN member practicesand all relevant stakeholders (ICB, Federation, Patients etc)

Develop andseek out opportunities to promote the PCN and the services it offers.

To represent the network at relevant external meetings

To foster a culture of collaboration and promote learning to drivecontinuous improvement.

To work as a flexible member of the PCN, providing support to other teammembers when necessary

To take an active role in the development and embedding of the PCN'sculture, values and reputation

To represent the network at external meetings

Buildrelationships and facilitate collaboration between practices within the networkto support the delivery of care to patients (, delivery of enhanced accessor other hub-type working arrangements)

The post holder may be requiredto undertake other duties should the post require it and will be required totravel to various locations on occasions to fulfil the requirements of therole.

Experience of working within a Primary Care Network, or GP Practice.

DBS check will be required upon appointment . Able to maintain confidentiality at all times. Able to fulfil the travel requirements of the role.

Qualifications

Essential

Good standard of education with excellent literacy and numeracy skills. Leadership and/or management qualification (or equivalent experience).

Desirable

Educated to degree or masters level in healthcare or business. AMSPAR qualification level 5 or equivalent
