Job overview

Job Summary
This post will be based at North Middlesex University Hospital (NMUH) to support the Lead Consultant 
for the upper GI 2 ww referrals. NMUH is running remote outpatient appointments and straight to 
test triage to establish patient pathways to facilitate diagnosis and hence definite treatment for 
patients with unexplained GI symptoms. CNS will be responsible for the management of these patients 
from GP referral, to include telephone triage, attendance at one-stop upper GI clinic, Consultant
review, timely investigations, rapid onward treatment or discharge. 
The CNS will be the first point of contact for patients referred to upper GI 2ww and be responsible for 
booking appropriate investigations and pre-assessment of the patient. The CNS will be accountable 
for co-ordinating appointments working closely with the administrative team within Gastroenterology 
and booking teams. Be responsible in ensuring essential follow up appointments are made to enable 
patient monitoring and ensure databases are maintained and attendance at MDTs for Upper GI/HPB. 
The CNS will provide expert clinical skills in assessment of the patient and support the patient on their 
pathway from referral to discharge. T
Gastroenterologist-to ensure appropriate investigations undertaken in line with protocols. To address 
patients’ health needs through planning and delivering interventions which are based on best practice 
and professional judgement. 

Main duties of the job

Duties and Responsibilities:

To be primary contact for patients referred for assessment within STT service and support the 
patient and carer on appropriate pathway. 
2. To receive electronic patient referrals via ERS 
3. Consultant review, where appropriate. 
4. Assist with the assessment of new patient referrals and working alongside Lead Consultant to 
ensure investigations are performed in a timely manner and an agreed management plan is in 
place.
5. To identify the physical, social and psychological needs of the patient with unexplained GI 
symptoms and suspected colorectal cancer. 

Management –
1. To collect, collate, evaluate information relating to 2ww upper GI and pre-assessment services. 
Patient pathways are tracked and data collated on key events such as timing of receipt of referral, 
date of diagnostic tests, delays and clinical outcomes. 
2. Present information to professional groups, analyse data and provide written reports. Data will be 
analysed frequently to identify risks or delays in patient care which require immediate 
intervention by CNS or service transformation. 
3. To establish and maintain effective communication links with all relevant personnel and 
departments, external agencies as necessary. 
4. Promote collaborative working relationships with other health care professionals for the benefit 
of quality patient care. 
5. To liaise with the health advocacy service to book advocates as appropriate to the patient’s needs.

Detailed job description and main responsibilities

To provide specialist clinical knowledge and practice to patients and carers within the Department 
and wards. Ensure the patient and family are fully informed and understand the purpose of any 
investigation and outcomes, in accordance with best practice and informed consent. 
2. To promote a patient focused multi-professional approach to care in collaboration with carers, 
health care professionals and other agencies. 
3. .Safely prepares patients for any investigation and facilitates the patient journey until discharge 
from Upper GI STT service . 
4. To pre-assess both outpatient & inpatients and use decision making to determine patient clinically 
fit for interventions and these are appropriate. . 
5. To communicate with the administrative team and confirm all appointments for diagnostic 
investigations / procedures are timely and as per protocol. 
6. To co-ordinate patient care including prescriptions and update patient and patients’ GP or 
referring Consultant of the care / treatment plan. 
7. To make decisions independently regarding clinical care and priorities where appropriate. 
Demonstrate clinical decision making and an ability to monitor and improve standards of care and 
safely remove patients from the 2ww pathway . 
8. To ensure the patient is informed of the outcome of their tests and they receive a definitive plan 
with referral to specialist / oncology team for those patients with a cancer diagnosis. 
9. To undertake aspects of the extended role of the nurse following appropriate training. Perform 
and assist with agreed procedures appropriate to the role and function of the Clinical Nurse 
Specialist 
10. Communicate complex information following assessments and analyse information which is 
sensitive to patients / carers and health professionals. This may be face to face assessments or via 
telephone consultations. 
11. Communicate information effectively and answer queries from patients, their relatives/carers and 
the multidisciplinary team members

Education:

1.To provide nursing clinical leadership and be responsible for the delivery of high quality patient 
care. 
2. Evaluate service and identify areas of improvement to be assured clinical and financial efficiency, 
effectiveness and resourcefulness. .. . 
4. Attend multidisciplinary team meetings and present individual care studies or relevant data on 
service.

standards of accountability, financial control, patient and public involvement and the delivery of 
clinical outcomes.
8. To provide Specialist nursing cover for those patients being cared for by associated CNS colleagues 
for Upper GI and STT. 
9. To support, enable, motivate, develop nursing staff and demonstrate Nursing leadership. 
10. Identify, develop and maintain ways to support patient involvement and feedback of service. 
11. To take an active lead in advancing nursing / clinical practice in research and implementing 
evidence based practice in designated areas.
12. In conjunction with multi-professional team, participate in setting objectives for service and 
engage with service improvement initiatives and strategies to deliver the FDS for upper GI 
pathway service. . 
13. Network within and outside of the organisation in relation to service development. Influence 
strategic developments in pursuit of advancement of quality of patient care, clinical outcomes and 
patients’ experience.

Registered Nurse First level degree in nursing or equivalent academic courses Teaching qualification or equivalent experience Masters level learning or willingness to work towards Knowledge of current clinical developments within speciality

Desirable criteria

Minimum 2 years experience working at band 6 level Experience of working in the acute medical setting within speciality of gastroenterology
