Weare looking to appoint a Occupational Therapist to join the Intermediate Care teamwhich is part of the Neighbourhood Rehabilitation Service (NRS) working acrossBarnsley Community.

The teamprovides rehabilitation to patients in their own home & residential care.The service provides specialist community therapy assessment & treatment toimprove health & independence. In addition, the service offersrehabilitation to enable earlier discharge from hospital or to prevent hospitaladmission.

You willwork within a multi professional team& hold a varied caseload providing therapy input & high-qualityrehabilitation to people within their own homes & local care homes.

You will assess, manage & provide treatment to individualswith complex and/or multiple needs, working closely with the MDT providingsupervision/support to other staff.

The team is committed to personal development & providesallocated time for CPD, training & wellbeing.

A vehicle to undertake travel is essential &the ability to work flexibly including evenings, weekends & bank holidays.

Main duties of the job

As OT you will:

Beresponsible for the assessment & provision of a specialised programme ofrehabilitation, meeting the specific needs & agreed goals of your patientsto facilitate their rehabilitation & optimise recovery potential.

Manage a clinical caseload with autonomy.

Demonstrate knowledge,experience and relevant clinical skills.

Monitor and evaluate treatment tomeasure progress/outcomes to ensure effectiveness of interventions and servicedelivery.

Develop programmes of care to supportdischarge.

Contribute to clinicalleadership/supervision facilitating a culture of learning/reflective practice.

Have high level communication and excellentinterpersonal skills to negotiate and establish collaborativepartnerships with patients and care professionals.

Ensurecompliance with clinical governance being responsible for ensuring InfectionControl policy/procedures are followed.

Job responsibilities

JOB SUMMARY

To provide a specialist OccupationalTherapy Service for the treatment of patients within the Integrated NeighbourhoodTeam.

Toprovide supervision, training and development of Basic Grade OccupationalTherapists, other health care professionals, support staff, and of OT students,if present.

To undertake high quality evidencebased assessments and treatments of patients within the relevant clinicalarea. These patients may have complexneeds and multiple pathologies.

To participate in planning, developmentand evaluation of Occupational Therapy services within the clinical areas.

To assess occupational performance,plan and carry out goal orientated intervention programmes.

To provide Occupational Therapy adviceto other professions.

To assist in administration,supervision and development.

KEY RESULT AREAS:

Toprioritise and manage a significant caseload of patients within the IntegratedNeighbourhood team.

Toassess for physical, cognitive and perceptual deficits in patients and to carryout assessment, treatment and using critical analysis to treat patients throughgoal setting, which is evaluated and adjusted throughout the patients period ofcare.

Toassess the individual in the most appropriate environment and to be responsiblefor the assessment, prescription and evaluation of specialised equipment,taking into account safe use/risk to patients and carers.

Toreceive and communicate complex, sensitive and contentious information in aclear and effective manner with patients/family and carers. To overcome barriers such as hearing loss,fear, behavioural issues, language problems in order to maximise rehabilitationpotential.

Tocollect and effectively communicate information to relevant agencies, actingfor and on behalf of, the patient and carer, in order to obtain equipment orservices, in order to maximise collaborative working.

Deputisefor Band 7 in the management of the services for duties identified by them.

Toattend meetings, provide detailed written reports, risk assessments, dischargesummaries and other relevant information and recommendations. To communicate effectively within theMulti-disciplinary team, contributing to the decision making regardingdischarge, planning and long term management.

Toparticipate in the induction and training of staff within multi professional,multi agency teams.

Toprovide formal and informal supervision of junior grades of staff andrehabilitation assistants.

Toundertake the education of Occupational Therapy students whilst on clinicalplacement within the department.

Tocomply with professional and trust policies and procedures, to take individualresponsibility towards health and safety issues, undertaking risk assessmentsas appropriate.

Tobe responsible for maintaining own continuing professional development in linewith trust policy and professional guidelines, and to undertake appropriatetraining and development to meet the needs of KSF/SDR.

Toparticipate in service developments in areas related to evidence basedpractice, service delivery and clinical governance.

Tocontribute to the Occupational Therapy team, joint therapy team,multi-professional team and older peoples service development.

Tobe responsible for maintaining accurate and comprehensive patient treatmentrecords in line with COT/BAOT guidelines.

Toprovide supervision, training and development of Basic Grade OccupationalTherapists, other health care professionals, support staff, and of OT students,if present.

Degree or equivalent in Occupational Therapy. Current HPC Registration.

Experience

Demonstrable post-graduate experience, in Older Peoples / Intermediate Care Services. Demonstrable experience of Evidence Based Practice and Risk Assessment.

Desirable

Experience of supervision of other staff. Experience of audit/research.
