Job summary

The post is a result of a collaborative cancer patient's improvement programme, aiming to provide people with cancer with an excellent experience of care, meeting the commitment to supporting cancer care, which is one of the Trust's top quality priorities. The post holder will play a key role in developing nursing expertise in skin cancer to improve the experience of local population requiring skin cancer care.

To work as an expert skin cancer nurse within cancer Multi-Disciplinary Team To manage a caseload of patients/clients effectively in accordance with agreed National and South East London Cancer Alliance (SELCA) guidance, protocols and pathways of care To ensure that patients and their families are fully informed and supported by providing clinical, emotional, psychological and social assessment and support in accordance with Personalised Care Standards To assist in the development of evidence based clinical nursing practice in line with current research and guidelines To establish clear lines of communication within the specialty team to ensure cohesive multidisciplinary management of patients To be integral part of established cancer team and specialist MDM, maintaining excellent communication to support coordinated care between sites To be act as a patient advocate at cancer MDM To link with healthcare teams in order to increase knowledge and facilitate the care of this group of people in the community

Main duties of the job

Fully comply with the trust Cancer CNS Standard Operational Policy To assist in the delivery of an expert specialist nursing service for people with cancer To provide specialist nursing advice, clinical assessment and support to patients, their families and other healthcare professionals following diagnosis and through treatment and where applicable onward referral for end of life care. To ensure continuity of a high standard of evidence based nursing care, assessing health related and holistic needs of patients, their families and other carers by identifying and initiating appropriate steps for effective care.

This can include:

o Proactive Managing of cancer patient caseload

o Initiating and interpreting tests and investigations used in cancer treatments, ongoing monitoring and late effects care

o Demonstrate sound knowledge of cancer and its treatment in relation to people living with and beyond cancer

o Symptom management for the consequences of cancer and its treatment

o Making and receiving referrals based on the needs assessment

o Develop practice to be able to deliver nurse led clinics supporting cancer patients care and needs

o Support the team and organisation in delivering innovative services to support cancer patient access to the team and early diagnoses

Job responsibilities

1. Clinical Nursing Practice

o Assessing and meeting patients on going care, health and wellbeing needs and develop a care plan and action referrals as a result

o Using data to identify high-risk individuals and groups and to ensure that services meet their needs

o Supporting Self-Care, Self-Management and Enabling Independence

To reduce utilisation of healthcare and improve quality of life for people leaving with cancer

Ensuring clear documentation in the patients records of all significant consultations in accordance with Best Records Keeping Guidance To be actively involved in inpatient care when appropriate, liaising with ward staff to ensure high quality nursing care Act as a key worker point of contact for patients cancer care related queries, ensuring that patients receive expert advice on their condition and treatment Ensuring CNS presence at all significant consultations where cancer diagnoses are communicated with all newly diagnosed patients Ensure all patients receive appropriate follow-up and that relevant information is available to enable patients to make an informed choice about their treatment and care Support patients with decision making about their care or treatment, ensuring shared decision making

Take responsibility for delivery of Personalised Care Key Performance Indicators (KPI`s) following the trust Personalised Care Guidance for delivery and submission of the data (Personalise care link: London personalised care KPI`s link: )

Regularly attending MDT meetings and acting as patient advocate, increasing visibility of patients to the cancer team, based on the individualised needs assessment Person Specification

Essential

NMC Level 1 Registered Nurse Educated to Degree level or above Relevant post-registration qualification and evidence of professional development

Desirable

Advanced Communication Skills Level 2 psychological skills

Extensive clinical experience pertinent to cancer care, oncology and specifically skin cancer Sound knowledge of Cancer Personalised Care and NHS Long term plan Experience of data collection for audits, analysing data and delivering presentations Experience of managing caseload of patients
