Main duties of the job

The post holder will assist in the overall delivery of care as determined by the lead practitioner; ensuring prescribed care is carried out according to the agreed care plan in conjunction with the multi-professional and multi-disciplinary teams. Social Inclusion workers will support the multi-professional team and contribute to the process of assessment and delivery of care needs to support and develop good clinical practice. There will be an emphasis on supporting the discharge of Service Users from the addition, the post holder will be responsible for providing information, support and advocacy throughout the Unit, to Service Users and Carers.

Job responsibilities

Under the supervision of the Pathway Co-Ordinator the post holder will:-

Work closely with multi- professionals carrying out specific tasks and direct work with service users and their families. Work with service users to support and enable them to function at their optimum potential, using the principles of the recovery model within a social inclusion framework. Provide and maintain a high standard of care in a way that respects the privacy and dignity of the individual service users to promote their independence. Promote ethically sound practice, including equality of opportunity and ant oppressive practice. Maintain a personal caseload ranging from information enquires to complex and ongoing emotional support and/or advocacy needs to Carers of people with Mental Health problems. Maintain strict confidentiality guidelines regarding all service users, conversations and referrals. Liaise with community agencies Statutory and Non Statutory including primary care and Benefit Agencies Work within your own sphere of competence recognising when there are times when you need to escalate issues that arise within your role. Essential

18 months experience working with individuals with mental health problems Awareness of Criminal Justice system and impact of this on service users with mental health problems Experience of and or training in risk management Experience of working with carers Experience or knowledge of providing welfare benefit information, advise and representation.

Effective verbal, listening communication and relationship skills Ability to work cooperatively as a part of a team and work autonomously in secure/forensic setting Able to work on own caseload with regular supervision Ability to work with different service users groups with mental disorder, offending behaviour and or challenging behaviour Ability to work with and be sensitive to and appropriately engage carers/families
