This role is a part of a pilot project funded by Stroke Quality Improvement Funds running for 12 months from March 2024-2025. It is a joint project with The Stroke Association. The role involves supporting patients post stroke who have undergone rehabilitation at the Sussex Rehab Centre to confidently transition into the community setting. The role involves providing ongoing rehabilitation to continue recovery until Community Neurological Rehab Services begin intervention. The role involves working with multiple Sussex NHS trust and Charitable teams.

Detailed job description and main responsibilities
Design rehabilitation programmes for Stroke patients under the guidance of qualified therapists
Implement and manage rehabilitation programmes prior to discharge from the Sussex Rehabilitation Centre and in community settings post discharge.
Work closely with all post holders funded by the project to deliver an effective transitional care model.
Work collaboratively with the wider Sussex Stroke Network to deliver a one team approach to rehabilitation.
Contribute to data collection for project evaluation through timely record keeping and administrative duties.
