A new and exciting opportunity has arisen within the Community Childrens Nursing Service. We are pleased to be able to offer a rotational position working within the Childrens Hospital @ Home Service and Community Children's Nursing Service at either full or part time hours.

We are looking for passionate and motivated Children's Nurses who have an interest in working within the community, supporting the provision of care for children and young people within their own homes preventing regular hospital attendances.

The role is a rotational position and will work closely with partners in acute trusts and primary care networks as part of an integrated health offer for children and young people. The care will be provided in children's homes, schools, nurseries and across other community settings.

The CH@H and CCN Teams work closely together and support each other as needed to deliver high quality care to patients. Both roles entail a high level of autonomy, independent caseload working and clinical decision making. You will be supported with regular supervisions, 1:1s and appraisals.

Main duties of the job

1.Professional/Clinical

*To support the Team Leader by carrying out specific health care assessments and/or interventions

*To carry out initial assessment and triage of patients with short term health care needs by telephone, at clinical settings or in patient's homes under the supervision of a Team Leader

*To provide continuity of care for children on caseload and maintain contemporaneous records accordingly

*To discharge patients according to pathways of care, under supervision of Team Leader.

*To support the provision of Community Children's Nursing service within special schools as service needs require, under the supervision of a Special School Children's Nurse.

*May carry out initial home assessments with guidance from the Team Leader, to plan, implement and evaluate programmes of care.

*To work in partnership with families and the multi-disciplinary team to provide care in appropriate settings to maximise life opportunities for the child or young person.

*Report any changes in the child's condition to the Team Leader and medical colleagues and ensure all records are maintained in accordance with Trust policies.

*Participate in the provision, storage and administration of medicines including intravenous therapy in accordance with Service/Trust guidelines and policies and in partnership with pharmacy colleagues

*To participate in MDT meetings to support children with complex health care needs.

About us

We have been working with other local NHS Trusts to create a new organisation, bringing together all community, mental health and learning disability services from across Hampshire and Isle of Wight. Subject to approvals, the aim is for the new Trust to be formed by the end of 2024.

Project Fusion is the name for the programme to create the new combined NHS Foundation Trust. Hampshire and Isle of Wight Healthcare NHS Foundation Trust will be bringing the services into a single organisation will result in more consistent care, more equitable access to services irrespective of postcode, and a more sustainable workforce and services. The new organisation will operate locally to ensure services can best meet the needs of different communities.

The new Trust will be comprised of all the services currently provided by Solent NHS Trust and Southern Health NHS Foundation Trust, the community, mental health and learning disability services provided by Isle of Wight NHS Trust and child and adolescent mental health services delivered in parts of Hampshire by Sussex Partnership NHS Foundation Trust.

Colleagues will transfer to the new organisation under Transfer of Undertakings (Protection of Employment) -- known as TUPE on the day the trust is formed. TUPE is a legal process to ensure you transfer out of one organisation and into another, maintaining the terms and conditions of your employment.

Job responsibilities

The service aims to provide high quality nursing care which enables early, safe planned discharge from hospital and prevents unnecessary admissions.

The Children's Hospital @ Home team provides specialist nursing care, support and co-ordination of care; predominantly to children and young people with short-term acute health care needs requiring urgent care pathways. Our care is led by Wessex Healthier Together. As a service we work closely with University Hospital Southampton. CH@H delivers service promotion to all referrers across the region including GP Surgeries, Children's Emergency Department, Paediatric Short Stay Unit and Paediatric Wards. Support is provided to children and families via telephone calls, video calls and home visits.

The Children's Community Nursing Team provides high quality holistic, family centred nursing care for children, young people and families with short and long term conditions, disabilities, complex health conditions, continuing care needs, life limiting and life-threatening illness, including palliative and end of life care needs with use of a 24/7 rota when required.

The Staff Nurse is responsible for the ongoing assessment of care needs and the development, implementation and evaluation of care. The provision of skilled nursing care for children, including those with palliative care needs in their own homes and giving advice and support to the family is essential in this role.

Our service is a link between Primary, Secondary and Tertiary care, Community Paediatricians, Therapists, Public Health Nurses, Education settings, Children's Services and voluntary organisations to support and encourage the development of partnership working and a seamless patient journey. Care is delivered in the community location that is the most appropriate for that child or young person home, school, respite centre with an objective to help the children and young people and their families achieve their full potential and live as full a life as possible.

The CH@H and CCN Teams work closely together and support each other as needed to deliver high quality care to patients. Both roles entail a high level of autonomy, independent caseload working and clinical decision making. You will be supported with regular supervisions, 1:1s and appraisals including access to the Duty Nurse via telephone whilst on visits to address any queries or concerns.
