To support patients and those important to them with a palliative life-limiting condition.

Job responsibilities

Assesses analyses and interprets complex facts or situations of patient conditions and family circumstances requiring analysis. Support and educate patient's families and carers and work in collaboration with the MDT at Fair Havens and within the wider community and hospital teams Prioritises workload along with support from senior members of the team and can take responsibility for own caseload Work to achieve our vision and values in respect of health and safety, inclusion, and the safeguarding of our people. Taking personal responsibility to comply with the principals/requirement To be responsible for conforming to the policies and procedures as set out in the staff handbook, including health & safety requirements and attending courses as directed Person Specification

Qualifications

Essential

Diploma or degree in Physiotherapy. Professional registration as recognised by HCPC.

Skills & Experience

Essential

Good standard of IT skills, including using Microsoft Office, Teams, Outlook and Excel. Experience of managing caseloads Experience in at least 2 discrete service areas, for example, physical health and rehabillitation.
