Main duties of the job

The post holder will be an experienced nurse or allied health professional, who acting within their professional boundaries, will provide care for the presenting patient from initial history taking, clinical assessment, diagnosis, treatment and evaluation of their care. They will demonstrate safe, clinical decision-making and expert care for patients within the general practice. Non-Medical prescriber The post holder will work collaboratively with the multi-disciplinary general practice team to meet the needs of patients, supporting the delivery of policy and procedures. The post holder will build effective working relationships with other health care professionals. The post holder will work within the multi-disciplinary team to deliver excellent primary health care services within the practice Clinical Expertise: Demonstrate expert clinical knowledge and skills in the assessment, diagnosis, and management of patients. This includes conducting comprehensive patient assessments, formulating treatment plans, and prescribing medications as authorised. Collaborative Approach: Work collaboratively with the healthcare team to provide holistic care and ensure effective coordination of patient services. This involves regular communication, consultation, and collaboration with physicians, nurses, and other healthcare professionals

Job responsibilities

The organisation and delivery of primary care services in ELFT is being re-visioned to reflect an aspiration to deliver services in a more integrated manner in collaboration with internal and external partners. To this end the portfolio of primary care services provided directly by the Trust is expanding and the model of service delivery is changing to include more direct participation of service users. Directly provided primary care services range from specialist homeless practices to mainstream general practices and are located across a wide and diverse geography including central London, Bedfordshire and Luton.
