Job summary

We are looking for a Patient Care Facilitator to start October 2024, due to retirement. The vacancy is for 20 hours over 4 days, the specific working pattern of this can be negotiated on appointment.

The role is part of a Facilitation of Admission Avoidance Scheme.

The applicant must be a well-motivated and proactive individual.

The aim of the role is to support patients by assessing their individual needs with delivery and support to access safe and effective services to improve outcomes and reduce avoidable hospital attendance or admission.

Main duties of the job

The job role will include (but not limited to) the following:-

- Manage own work load

- Contact patients to arrange appointments. This can be face to face at the GP Practice, Home Visit, telephone or video call, depending on the patient's preference

- Develop strong links with the Primary Care Network Social Prescibing Link Workers to work together to identify suitable support links for patients including community based resources, including volunteers and charitable organisations and actively sign-post patients to these organisations where appropriate

- Act as a practice point of contact for patients and their carers

- Maintain information included within the practice resource pack and review on a regular basis

- Ensure patient and carer feedback is collected to help develop patient-led services in line with local needs

- Engage with the Practice PPG members on a regular basis

We are also a committed training practice. We currently have a GP Registrar, Foundation Year 2 Doctor and two medical students on placements at the practice.

In addition to the practice based team, as a member practice of Newcastle North Primary Care Network, our practice team and patients benefits from the support of the wider PCN team. This includes Pharmacists, Pharmacy Technicians, Dietician, Mental Health Practitioners, First Contact Physiotherapist, Paramedic, Care Home Team, Social Prescribers, Health and Wellbeing Coach and Care-Coordinators.

Job responsibilities

Create and maintain a patient register Invite patient cohorts for a health check in their home, at the surgery, by telephone or by video Undertake health checks in order to identify assistance that could be offered to avoid hospital admission Liaise with appropriate organisations and following up referrals social services, OT, Social Prescribers, DWP, Fire Service, GPs, nurses, district nurses, pharmacies etc Show compassion and understanding to patients and their families Demonstrate knowledge of Power Of Attorney and ensure/ check that consent is in place for family, should the need arise to contact family members. Ensure all the leaflets in the information packs are up to date and correct at the time of distribution Demonstrate knowledge of what to do in an emergency during a home visit Keep up to date with benefits ( pension credit, carers allowance, attendance allowance) Liaise with other members of staff to gain knowledge which may help to understand patient situations or medical problems Ensure information in consultations is correct and a true reflection of the home visit or phone call Collate feedback questionnaires to inform service design Contact patients or relatives to arrange appointments Create and provide information packs to the patients that will inform them of local services, support groups etc. including sourcing leaflets for new services Discuss at clinical meetings on a regular basis any recent assessments, outcomes or feedback received from patients Attend any training courses that are suitable for the role Arrange follow-up, referrals to secondary services, GP appts or other appropriate clinical staff and follow up the patients post referral Ensure that you keep in touch with the Practice while out on calls and to remain safe at all times To meet or discuss any issues regarding patients or the scheme with a GP/NP or PM if required Create a robust patient recall process tailored to the individual patients needs Create care plans when appropriate for patients

Experience

Essential

Experience of dealing with vulnerable patients and their families. Understanding of Frailty and Rockwood Scoring System. Ability to determine own workload priorities. Excellent communication skills.

Experience of working in primary care. Social Care background.
