Our 21 bedded unit provides rehabilitation to a wide group of patients with varying complexities of needs. Our patients come to us following medical, surgical or orthopaedic treatment or if appropriate, directly from their own homes. We also provide palliative and end of life care.

Main duties of the job

Implement care for patients as defined by the individualised care plan. Provide a high standard of direct care for patients. To work in a collaborative manner with all other staff working within the establish a good relationship with the patients and provide an appropriate support mechanism so the patients are able to express their needs. To ensure the patients are involved in the decision making process, communicate all relevant information regarding the needs of the patients, family and friends. To document and sign all care interventions provided. To be involved in the induction and mentoring of new staff where applicable.

Ensure effective and efficient use of all ward/department resources. Maintain a positive approach towards achievement within the team. Ensure adequate provision and maintenance of equipment. Participate in problem solving that is related to your work and organisation. To ensure that patients and carers are aware of their current and ongoing health needs. To participate in and promote clinical supervision.

Main Duties & Responsibilities

To accurately and promptly carry out the instructions of the Registered Nurse (RN) who is professionally accountable for the delivery of care. To carry out duties as specified within the trusts Approved Core Task List (page 5) for Clinical Support Worker (working within assessed competence) To report to registered staff any patient that gives cause for concern or any observed changes in a patients clinical condition. To support patients emotional well-being by supporting positive conversations, actively encouraging participation in ward-based activities To accurately document all care delivered to patients, including recording documentation electronically on trust systems maintaining confidentiality at all times. To respond appropriately to clinical emergencies to ensure the patient receives appropriate care. Be aware of role in emergency situations and what actions should be undertaken. To be aware of and adhere to all trust policies / procedures / nursing guidelines when undertaking any activities. To make patients welcome to the clinical area and participate in the admission process under the supervision of registered staff. Provide and deliver a high standard of individualised personal nursing care. To keep the clinical area clean and tidy maintaining a safe environment To adhere to trust infection control procedures. To assist in the preparation of patients and equipment for treatment, investigations or procedures as directed by a RN. To chaperone/escort patients to other clinical departments and or NHS Units under the direction of the RN. To support relatives under the guidance of a RN. To assist with the discharge process for patients directed by RN and or member of Multidisciplinary Team (MDT)

Management/ Organisational Responsibilities

Ensure effective and efficient use of all ward/department resources. Provide leadership to all members of the ward, promoting effective team work. Be responsible for monitoring staffing levels and the skill mix to ensure that this is appropriate to meet the needs of patients, and taking initial action to rectify any identified deficits through ward manager/modern matron. Maintain a positive approach to achievement within the team. Ensure adequate provision and maintenance of equipment. Assist with the induction of new staff and students, acting as a mentor. Act as a role model giving guidance and support to other staff and students. Participate in problem solving related to work organisation.
