Job summary

We are looking to build junior sister workforce for our new frailty unit at Morriston Hospital. This is an exciting opportunity to work as a senior member of the leadership team within the frailty unit. Providing care to patients presenting with, injuries, illness, acute exacerbation of chronic conditions.

We require someone who is dynamic, innovative and highly motivated role model with strong leadership attributes, with good communication skills to support the team.

The unit is opening on the 1st September 2024.

Main duties of the job

Main Duties

Candidates need to be able to demonstrate good clinical and assessment skills and able to manage the team in the absence of the unit Senior Sister. You will be responsible for leading and managing a team of staff, while working closely with the multidisciplinary team to shape the new unit and frailty strategy.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Frailty Strategy overview

The Current Situation

Within Swansea Bay UHB we recognise that frailty is associated with increased ill health, mortality, healthcare use, care dependency and reduced quality of life and have identified a high proportion, 40%, of our acute medical admissions are frail.

People with frailty often experience long waits in overcrowded admission areas and spending extending periods of time in these types of environments is challenging to their senses and increases immobility which leads to deconditioning and delirium. In an attempt to move people quickly out of admission areas like ED (emergency department) we are not always able to offer older people a frailty assessment before they are admitted. By failing to complete a frailty assessment (ideally within 30 minutes of their arrival at hospital) patients are at greater risk of hospital acquired harms.

What can we do to provide better care to our frail population?

We want to provide a dedicated Frailty Unit that is staffed by a multi-disciplinary team of geriatricians, nurses and therapists who will be able to provide the targeted care these vulnerable patients need, right from the offset. By undertaking a comprehensive geriatric assessment on arrival as well as having six ring fenced beds for patients for those confirmed with fractured neck of femur and enabling specialty in reach and continuity of care across admitting areas and wards we can minimise the time spent in hospital and reduce unnecessary admissions.

Person Specification

Qualifications and/or Knowledge

Essential

Registered Nurse (Part 1) - Active NMC Registration Bachelor degree in relevant health related subject (or equivalent experience) Pertinent postgraduate diploma (or equivalent development / experience and a willingness to undertake appropriate post graduate diploma level study) Knowledge of Safeguarding Adults/Children including the Mental Health Act and Mental Capacity Act and Deprivation of Liberty Safeguards Knowledge of quality and service improvement methodologies Bronze IQT Able to demonstrate sound knowledge of NMC Code (2015)

Desirable

Teaching qualification (or equivalent experience) Recognised management qualification Understanding of wider nursing and health care related agenda

Experience

Relevant clinical experience Experience of working in a multi-professional environment Experience of shift leadership Experience of working with preregistration nursing / midwifery students Experience of formal /informal teaching and of mentoring preregistration nursing or midwifery students. Able to relate theory into practice through reflective skills
