If you are lookingfor a role where you can use your skills to make a difference come and join ourteam where Nurses, Physiotherapists, Occupational Therapists, Health Care Assistants,Advanced Clinical Practitioners, Nursing Associates and Administrators worktogether in local Neighbourhoods and alongside GPs to support people to live intheir own homes.

Main duties of the job

The role will require:

The ability to communicate effectively withothers at all levels Working flexibly to provide the servicedelivery need across the local area (the service operates from 7:30am 6:30pm,Monday to Sunday including bank holidays) An enthusiastic, motivated and innovativethinker to become part of a primary care facing community The ability to work autonomously and workingwithin a multi-disciplinary team The ability to problem solve and manage timeeffectively Experience of working in a fast paced andvaried environment The ability to travel to deliver care servicesyou will have the use of a Trust pool car when allocated on a roster basis.

Qualifications

Essential

Level 3 qualification ( NVQ 3, A-level) in Care Or Level 2 qualification ( NVQ2, GCSE Grade A-C) in Care and will be expected to undertake the relevant Level 3 qualification on appointment to post Will be expected to meet the Care Certificate standards within 12 weeks of commencing employment with the Trust I understand that I will be asked to provide original certificates to verify my qualifications at point of offer.
