About the Role:

You will be working in a supportive team environment to deliver high standards of care and support to our residents. Your role will involve working closely with the care team to monitor residents' wellbeing, identify and address any changes in their care needs, and provide written records of any variations. You will also be involved in the person-centred care planning process, implementing and monitoring care plans, and conducting monthly evaluations. Additionally, you will be responsible for supporting residents with their medication requirements and undertaking staff supervisions to support staff development. You will be the key point of contact for residents, their relatives, and visitors, as well as health and social care professionals, ensuring that any reasonable requests are acted upon and concerns or complaints are escalated.

About You:

To be successful in this role, you will need to live our values of caring, supportive, honest, respectful, and accountable in all that you do. You will require an NVQ 3 or equivalent in care for care homes, 2 years' experience working in a similar care environment, and the ability to communicate effectively both verbally and in writing. You will also need to have a positive attitude to supporting and nurturing residents and staff, and be responsive and flexible to cover a range of responsibilities.
