Job summary

We are looking for a Nursing Associate to do this role aspart of a Primary Care Network (PCN) multi-disciplinary team. The post holderwill mainly focus on supporting patients with multi-morbidities such asdiabetes, chronic kidney disease and hypertension. They will be responsible forensuring the effective holistic provision of high-quality nursing care topeople in a clinic and outreach environments who are registered with practicesin Burmantofts, Harehills and Richmond Hill.

Main duties of the job

This exciting role with see the Nursing Associatesrole actively contribute to a patients health and wellbeing through thedelivery of holistic care, a whole-person approach which considers, and equallyvalues, physical, psychological, public health needs, social economic, spiritual,and other factors in the assessing, planning and delivery of care. Providingall relevant competencies (to be developed in line with the requirements of theservice) have been achieved, they will have delegated responsibility for apre-agreed caseload under the supervision of a nominated clinician within thePCN multi-disciplinary team. The post holder will be integral to the team ofclinical pharmacists, specialist Mental Health HCA, care coordinators: formental health, care home patients and children, GPs, Nurses, Health andWellbeing Coaches, Social Prescribers, and the wider local care partners suchas Community services, hospitals, specialists for long term conditions alongthe patients pathway.

How you will tackle health inequalities in BHR PCN, caring for patients with three or more long-termconditions and mental health (multimorbidity) is sadly very common and is aknown priority for BHR PCN. We think this is achallenging but extremely rewarding opportunity for a nursing associate to joinour committed and dedicated team who tackle these health inequalities daily.Our population have a higher prevalence of mixed mental and long-termconditions because of the nature of our more deprived communities, particularlyat an earlier age. Along with our culturally diverse population, thesemultimorbidity patients are typically our hard-to-reach patients.

To work within the Nursing Associate parameters ofpractice and deliver high quality, compassionate care to patients in their own homes/clinicenvironment.

To work with PCN colleagues to deliver care whichwill be focused on patients who have or are at risk of multimorbiditys byensuring they are monitored and have access to receiving the physical,psychological, and social assessments.

To work under the direction/delegation of anominated clinician, in accordance with an individual care plan, with a focuson promoting health and independence in a personalised care plan using the Whatmatters to me approach.

To carry out specific delegated clinical and caretasks and responsibilities to a high standard and competency, under thedirection and supervision of a registered nurse or other registered healthprofessional.

To proactively monitor and prioritise patientsappointments and visits within a delegated caseload, making autonomousdecisions within agreed competencies about patient care and seeking supportfrom colleagues as appropriate.

Work in an effective and organised mannerdemonstrating excellent time management and organisational skills toeffectively deliver person-centred care for an allocated group of individuals.

To contribute to the development of staff withinthe service through training and supervision.

- Experience of working in clinical teams under appropriate supervision including as part of a multi-disciplinary team. - Experience how to evaluate own strengths and development needs, seeking advice where appropriate. - Community experience. - Experience of providing and receiving complex, sensitive information. - Knowledge and understanding of skin integrity (to appropriate level)

Skills

Essential

- Ability to undertake and understand physiological measurements and observations pulse oximetry, peak flow, urinalysis and general observations, phlebotomy. - Ability to communicate effectively (written, verbal and non-verbal communication) with patients/relatives and carers and all members of the multi-disciplinary team. - Evidence of IT and keyboard skills, including use of PC, IPads and other associated IT equipment. - Understand importance of infection prevention and control inc. PPE, ANTT, sharps. - Ability to take part in reflective practice and clinical supervision activities. - Evidence of time management skills and ability to organise and prioritise own delegated workload. - Ability to deal with non- routine and unpredictable nature of workload and individual patient contact. - Ability to develop effective and appropriate relationships with people, their families, carers, and colleagues. - Car driver with a current UK licence and access to a car for business purposes.

Qualifications

- Nursing Associate Level 5 foundation degree qualification - GCSE Grade A-C in Maths & English or skills level 2 Maths & English or equivalent

- Registered Nursing Associate on the NMC register

Knowledge

Essential

- Understanding of the scope of the role of the Nursing Associate in context of the nursing and interdisciplinary team and the organisation, and how the role may contribute to service development. - Understands and acts in line with NMC professional standards for practice contained within The Code and revalidation. - Knowledge of when to seek advice and escalate to the appropriate professional for expert help and advice. - Understanding of the importance of the promotion of health and well-being. - Demonstrates commitment to continuing professional development through further study. - Understanding of the importance of the promotion of health and well-being. - Understanding of evidence-based practice. - Knowledge and understanding of Safeguarding to appropriate level.
