Job summary

Working with the Torquay Primary Care Network, this role would see a Pharmacist working within the Brunel Medical Practice Team.

Training, support and continuing personal development facilitated by our Senior Pharmacist.

Successful candidate is required to undertake the independent prescribing course to become a non-medical prescriber if not held.

Management and leadership development training supported in-house by Practice Staff, GPs and the wider practice team.

Main duties of the job

We are looking for a motivated, enthusiastic and professional individual with a good clinical background, who is organised and confident to work independently and as part of a team, with the ability to prioritise workload. Applicants will be computer literate, have excellent communication skills. Knowledge of EMIS, Eclipse and PINCER would be an advantage as would experience of working within Primary Care. About us

The Torquay Primary Care Network is comprised of Brunel Medical Practice, Chelston Hall Surgery, Croft Hall Medical Practice and Southover Medical Practice.

This role would be working exculisvely for Brunel Medical Practice.

Job description

Job responsibilities

Key responsibilities for clinical pharmacists in delivering the health services to patients will be to:

i. Work as part of a multi-disciplinary team in a patient facing role to clinically assess and treat patients using their expert knowledge of medicines for specific disease areas.

ii. Prescribers, or will be completing training to become prescribers, and will work with and alongside the general practice team. They will take responsibility for the care management of patients with chronic diseases and undertake clinical medication reviews to proactively manage people with complex polypharmacy, especially the elderly, people in care homes, those with multiple co-morbidities (in particular frailty, COPD and asthma) and people with learning disabilities or autism

iii. Provide specialist expertise in the use of medicines whilst helping to address both the public health and social care needs of patients at practice and to help in tackling inequalities.

iv. Provide leadership on person centred medicines optimisation (including ensuring prescribers in the practice conserve antibiotics in line with local antimicrobial stewardship guidance) and quality improvement, whilst contributing to the quality and outcomes framework and enhanced services. Through structured medication reviews, clinical pharmacists will support patients to take their medications to get the best from them, reduce waste and promote self-care.

v. Have a leadership role in supporting further integration of general practice with the wider healthcare teams (including community and hospital pharmacy) to help improve patient outcomes, ensure better access to healthcare and help manage general practice workload. The role has the potential to significantly improve quality of care and safety for patients.

vi. Develop relationships and work closely with other pharmacy professionals across PCNs and the wider health and social care system.

vii. Take a central role in the clinical aspects of shared care protocols, liaison with specialist pharmacists (including mental health and reduction of inappropriate antipsychotic use in people with learning difficulties), liaison with community pharmacists and anticoagulation.

viii. Be part of a professional clinical network and will have access to appropriate clinical supervision.

Masters Degree in Pharmacy (MPharm) or equivalent. Must have completed, be enrolled on or granted exemption from the CPPE Course.

Independent prescriber or working towards/intent of gaining independent prescribing qualification.

Experience

Essential

An appreciation of the nature of GPs and general practice. An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing. Excellent interpersonal, influencing and negotiating skills. The ability to work under pressure and meet deadlines. Produce timely and informative reports. Work effectively independently and as a team member.

Desirable

A minimum of 2 years post qualification experience. In depth therapeutic and clinical knowledge and understanding of the principals of evidence based healthcare. Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions. Gain acceptance for recommendations and influence/motivate/persuade the audience to comply with the recommendations/agreed course of action where there may be significant barriers.
