A new role has become available within The BaNES Recovery Team.
We are a friendly and supportive team who are passionate about improving the lives of service users with complex mental health needs. As a care co-ordinator in the team you will be responsible for a caseload of service users with a range of complex mental health needs, working within the CPA framework to deliver a range of health and social care interventions.
We strongly encourage personal and professional development - and once you are established within the team, there would be opportunities to undertake training to support you within the role, for example, SCM, DBT, MBT, understanding psychosis and family interventions.
We are looking for any profession with a registration to apply, including Social Work, Clinical Associates in Psychology, Occupational Therapists and Registered Mental Health Nurses.

Main duties of the job
Provide a Mental Health service to an allocated caseload of Service Users, and deliver a set of defined interventions within that caseload, having responsibility for the assessment of needs, the planning and implementation of treatment and care to meet those needs as agreed with multidisciplinary team and/or supervisor, coordinating care (with supervision) for particular Service Users, and contributing to the evaluation of care across a variety of settings where Mental Health or Specialist Treatment is available.
Liaise as appropriate with other members of the multidisciplinary team and other agencies involved in the care of individual service users.
Participate in Integrated Governance processes, particularly as they relate to own practice, contributing to the monitoring of service provision and performance, and standards of care, through data collection and audit processes.
Liaise with other services and agencies involved in provision of services to people with Mental Health and other vulnerabilities.

This role is focused on providing appropriate, effective interventions and treatments to people with severe and enduring mental health needs, and their carers (including friends and relatives) and their supporters in the community, enabling and assisting them to meet daily health, social care and well being needs, in line with personal recovery goals, and facilitating engagement with mainstream services.
Always under the overarching framework of CPA, the post holder will be responsible for the ongoing assessment, planning delivery and review of activities and interventions against identified health, social care and well being needs, acting as care coordinator for a defined group of service users.
We are looking for a staff member who is committed to excellent clinical care and who wishes to learn and develop new skills within the team.
You must have good communication skills and be willing to work independently, with the support from your colleagues and the multi-disciplinary team.
We are willing to look at newly qualified staff or graduates. There will be opportunities to access in-house and external training.
The focus of the team is to work within the recovery model. You will receive regular supervision and have support from your clinical supervisor as well as the senior practitioner in the team. We cover a wide geographical area and actively promote remote working to reduce travelling times.
Develop and maintain as far as possible effective helping relationships with service users, working in partnership with service users and carers and ensuring that your practice accords with guidance and best practice standards regarding the delivery of services including, but not limited to, requirements in relation to Privacy, Dignity and Respect and Diversity.
Provide a defined Mental Health assessment and interventions to an allocated caseload of service users, having responsibility for the assessment of needs and the planning and implementation of treatment and care to meet those needs, based on evidence of best practice, and as agreed with multidisciplinary team and/or supervisor, and contributing to the evaluation of care.
Working with service users, and others as appropriate, working collaboratively in the ongoing assessment of risk, and the development and review of plans to manage identified risks effectively, working closely with senior members of the multi-disciplinary team and practicing in accordance with Trust and National policy and best practice guidance.
Co-ordinate care for designated service users on your caseload under the supervision of a more senior practitioner
Develop and maintain good working relationships and effective communication with other members of your designated Mental Health or Specialised environment, participating in regular team meetings and contributing to peer supervision and training as appropriate.
Maintain accurate and up to date records of casework with individual service users, ensuring that your practice and record-keeping complies with all relevant standards and Good Practice Guidelines.
Ensure the monitoring and evaluation of the Service by collecting and recording high quality and timely data using electronic and paper systems as required.
Participate in your own supervision. Work with your line manager to set and review your objectives, and to identify and meet your training and professional development needs through supervision and appraisal against the KSF outline for your post. Maintain professional registration as appropriate, utilising time allocated through agreement with line manager and clinical supervisor to ensure professional development.
Ensure you keep up to date with developments in the Mental Health and Social Care field and related areas as appropriate, ensuring that your practice is based on best available evidence and meets all relevant standards and requirements set out in Integrated Governance frameworks.

Ensure that you maintain familiarity and work in accordance with all relevant National and Trust Policy and Guidance, including how and where to access this.
Examine and review personal and teams understanding of provision of equal access to services by diverse groups in line with AWP policy, facilitating fair and equal access, particularly to underrepresented groups.
Contribute to the provision of training for students.
Have responsibility for other duties as agreed with line manager.

This post is open to newly qualified social workers. In line with AWP trust policy all newly qualified social workers (NQSW) will be recruited initially at Band 5 level, regardless of advertised role banding. All NQSWs are expected to successfully complete their ASYE within 2 years of employment, unless there are exceptional circumstances in which case it may be extended for a further 2 years.
If you have been appointed to an advertised Band 6 role this will be an automatic progression, following confirmation of your successful ASYE completion. If you have been appointed to a Band 5 post you will then be eligible to apply for a subsequent Band 6 vacancy upon successful ASYE completion.
