About Us:

Role Overview:

Our Services in Chester and Winsford /Cheshire West mainly provides support services to Semi-independent care leavers aged 16-8. The Service support the young people to develop independent living skills such as cooking, cleaning and personal hygiene. Managing budgets and understanding the wider benefits of work is also critical, and the young people access programmes for education, training and employment.

We are recruiting to fill a Young People Progression Coach role at our new service in Chester and Winsford, with flexibility of working between the two services. The appointed candidates will join the team on a full time (37.5 hours per week), permanent basis with flexible rota that includes evenings, weekends, sleep-ins and bank holidays.

As Young People Progression Coach, you will undertake both lone working and working within a team responsible for delivering an assessment, support and move on service to care leavers in supported accommodation so that they can develop the skills and resilience to be confident and self-determining in their lives.

You will manage a caseload of young people, addressing their physical, emotional, social and behavioural needs. This will involve running individual and group support sessions and other key activities within the supported accommodation to help them progress towards independence.

Key Responsibilities:

Complete full assessment of each young person’s strengths, needs and goals to develop a holistic and strengths-based support plan and tailored programme of support. Provide a programme of support with young people to cover accommodation needs, independent living skills, support with education, employment and learning and social and emotional issues. Support young people aged 15+ to prepare for independent living and develop necessary skills. Actively support young people to stay in or enter suitable education, employment and training. Support young people to access and liaise with health services, including mental health services, drug and alcohol support services and counselling referrals.

What We Are Looking For – Person Specification:

Able to work on own initiative but at same time able to exercise sound judgement in knowing when to consult with peers and clients to meet the service KPI’s A clear understanding and ability to implement risk management, information sharing and data security arrangements. Experience of helping Young People with setting up benefits, developing independent living skills and maintaining their accommodation Experience of working with young people who may have complex needs such as mental health issues, drug and alcohol misuse, domestic abuse and offending behavior.
