Our Team is an ever-growing community that provides services from Cystic Fibrosis, Play Therapy, CYP Continuing Care, Palliative Care and Transition.

Our in-house Education Team ensure you are confident and competent in practical and delegation of skills including technology dependency equipment, complex care and moving and handling. Our nurse led out of hours service offers guidance and confidence throughout the 24/7 service.

Main duties of the job

To work as part of the education locality within the Community Children's Nursing Team promoting evidence based practice to all staff, children, young people, their families and other agencies to ensure quality care is delivered to children and young people within the community setting.

To promote and provide high quality standardised, holistic care throughout the community that will enable children and young people with complex healthcare needs to access all the settings they need to live a fulfilling life to promote their health, wellbeing and independence.

Job responsibilities

To maintain a professional attitude at all times, developing the role of clinicaleducator within the Community Childrens Nursing Team, (CCNT) in the bestinterests of the profession and in accordance with the NMC Code of professionalconduct. Also providing on-going evidence of eligibility to undertake relevant clinical activities to maintain own clinical competency andto support staff in developing clinical ensure that evidence based practice is developed and implemented tomaximise outcomes for children & young people.Initiate and support learning opportunities to meet the educational and skillsdevelopment needs of nurses, healthcare support workers, parents and children& young promote individuals equality, diversity and rights in accessing high qualitynursing facilitate training to enable staff from health, education and social services tobe competent to meet the children and young peoples care needs.

To participate and deliver induction/education programmes for staff, studentnurses and multiagency support others to develop and apply their knowledge and provide a rolling training programme of education and support for staff caringfor a specific group of children/young people. Professional / Clinical Responsibilities: In conjunction with the community childrens nursing team support thedevelopment and training needs of nurses, clinical support workers andwithin designated clinical areas according to individual needs and servicerequirements.

Facilitate learning opportunities to meet the educational and skillsdevelopment needs of nurses, healthcare support workers, parents andchildren & young support the development, review, updating and implementation ofcompetencies (using the Coventry and Warwickshire Children andYoung Peoples Interactive Competency Framework where possible) toensure evidence based practice is utilised at all times to provide highstandards of teaching to support high standards of review and develop relevant policies and procedures, care pathways,protocols and guidelines to ensure up to date evidence based practice.Participating in clinical audit as required. Ensuring nursing procedurestaught are in accordance with Trust participate in the Childrens Policy & Approval Group (CPAG) support the development and implementation of preceptorshipprogrammes for newly appointed work in partnership within the multidisciplinary team to facilitate, aprompt and efficient discharge from hospital, subsequently reducinghospital length of stay and unnecessary re-admissions where possible.Participate in the Childrens Continuing Healthcare maintain confidentiality as a priority, in all areas of work, withinprofessional boundaries as directed and expected within the multidisciplinary maintain individual, accurate, up to date healthcare records as pertrust establish effective communication and good working relationshipswith multi professional and multi-agency colleagues within the Trust tosupport effective service promote working in partnership with the child / young person theirfamily, Consultant Paediatricians, Nurse Consultants and key agencies,from both the statutory and voluntary sector.Develop own and others knowledge and practice across professional andOrganisational boundaries by providing clinical advice as required.Participate in Induction, training and preceptorship programmes forqualified nursing staff, including student nurses, healthcare supportworkers and as agreed with other agencies.Ability to work varied shifts as required to support training withinpackages.Following a period of induction to participate in telephone out of hoursadvise service and 24/7 support when delivering an episode of activepalliative care on call.Contribute to monitoring quality and the investigation of and maintain professional knowledge in relation to your area ofinterest around the national and local modernisation agenda for children,young people and their be aware of own learning needs and limitations and to maintainprofessional expertise by embracing the concept of lifelong work alongside the clinical lead to support the operational running ofthe CCNT ( Sickness management, return to work interviews etc.).Update and maintain professional knowledge around the national andlocal modernisation agenda for children, young people and theirfamiliesTo lead/assist with research and development when required byassisting local research by the Warwickshire Area Child Protection Committee and organise, facilitate and participate in multi-agency/disciplinarymeetings, as contribute to the women and childrens services management responsibilitiesAct as a clinical role model for effective clinical care enabling andfacilitating others to take forward provide clinical supervision and support to staff, one to one and groupsupervision and carry out personal development reviews as undertake personal development reviews of allocated members ofstaff.Participation in the development and delivery of induction, training,competencies and mentorship programmes of students and staff participate in the recruitment, selection and retention of organise and support student nursing and medical placements withinthe network and work collaboratively with clinical education teams bothlocally and nationally, including other statutory and voluntary agencies;to maximise development opportunities.Maximise opportunities to share good practice and innovations withcolleagues locally and nationally. Measureable Results areas To undertake risk assessments, complete the necessary documentationand take appropriate action including informing your line manager.Utilise all resources efficiently and effectively within the post holder will maintain accurate and contemporaneous records,deal with highly sensitive information respecting confidentiality andsecurity at all times in accordance with Trust policies and data be involved with and/or support staff in auditing services and carepractices on clearly identified areas as per Trust audit ensure that all clinical incidents and national patient safety alerts arereported, actioned and reviewed in a timely be responsible for and provide evidence that all mandatory training isundertaken & up to date as per Trust training post holder will be required to use a computer, either stand alone oras part of a networked system, and will be responsible for the quality ofinformation. The amount of time spent on this type of work will dependon the current needs of the caseload.Communications and Working Relationships:To ensure CYP obtain information and optimal treatment, taking intoaccount social, cultural develop and maintain effective communication networks with serviceusers, to support the enhancement of service delivery.

To develop and maintain an effective and comprehensive communicationnetwork within integrated childrens services (health, social care,education, acute care, hospices and the voluntary sector) and otherexternal agencies to ensure the best service for CYPs and their promote and advocate for the equality, diversity and rights of children& young people who access the service.Demonstrate sensitivity towards CYP and their families by respecting theprivacy of their home environment and maintaining professionalboundaries at all times.

Current NMC Registration ENB 997/998 or equivalent

Desirable

First level degree/specialist practitioner equivalent experience or willingness to work towards degree level (appropriate role) Nurse Medical Prescribing qualification or willingness to undertake.

3 year post registration experience. Evidence of recent continuing professional development Experience of caring for children with complex and palliative care needs. Evidence of multi-disciplinary working

Minimum 12-18 months working at Band 5 level in the care of sick children.
