Main duties of the job

To promote and sustain the independence of children & young people (CYP) with a range of complex healthcare needs by delivering skilled nursing care within the community in a variety of settings.

To undertake assessments of care, develop, implement and evaluate care programmes, in partnership with the child, young person and their family. To carry out managerial responsibilities for an identified caseload and skill mix of staff.

Job responsibilities

To assess, plan and implement care to meet the identified individual healthcare needs of CYP, and families (this will include provision of continuingcare support).To work in partnership with other care providers to facilitate, a prompt andefficient discharge from hospital, subsequently reducing hospital admissionsand unnecessary re-admissions.Working in partnership with the family, Consultant Paediatricians, NurseConsultants and key agencies, from both the statutory and voluntary participate in the risk analysis and decision making process at childprotection meetings, providing written reports and giving health advicewhere needed.Initiate and develop flexible and innovative approaches to meet the needs ofCYP and their families.Following a period of induction to participate in telephone out of hoursadvise service and 24/7 support when delivering an episode of activepalliative care on maintain a professional attitude at all times, developing the role withinthe Community Childrens Nursing Team, (CCNT) in the best interests ofthe profession and in accordance with the NMC Code of professionalconduct. Also providing on-going evidence of eligibility to assess and identify equipment which is suitable for CYP and familyneeds following the medical devices and procurement policy.

To be flexible to work within a 24 hour shift pattern, including weekends and bank holidays.
