Job summary

We are looking for an experienced and passionate Clinical Lead to join our Community Nursing Team based from our Canterbury locality.

Our Community Nursing Teams provide interventions for housebound patients aged 18 and over delivering personalised nursing care to those with complex and long-term conditions.

The aim of our service is to enable patients to stay safe and well in their chosen environment. The service actively works to preventavoidable admissions by providing tailored care that meets the patient need and assists them in actively achieving their long-term health goals and self-care where possible

Main duties of the job

As the Clinical Lead, you will be responsible for the management and leadership of individual staff members and the quality of the care they deliver. Your team(s) will be aligned to a Primary Care Network (PCN), where you will represent community nursing.

The role requires an experienced Community Nurse to provide sound clinical judgement and act as a role model for the profession. As the senior clinician you will be a decision-maker and a problem solver withaccountability for your team's performance.

Person Specification

Qualifications

Essential

Registered health care professional (NMC or HCPC registration) / minimum of five years' experience in health improvement environment. Health related degree and evidence of relevant post graduate development at Level 6. Evidence of Post Graduate Specialist qualifications relevant to area of practice (examples: non-medical prescribing, mentorship, coaching, clinical expertise). Management or supervisory qualification / programme of study.

Desirable

Preparing to Teach in the Lifelong Learning Sector / Diploma in Teaching in the Lifelong Learning Sector, Post Graduate Certificate of Education or Certificate in Education. Working towards Master level study

Experience

Essential

Minimum of five years' experience in area of practice. Experience of leading and managing teams. Experience of developing policy and service development plans with practical experience of change management. Practical experience in managing health related projects/complex caseloads. Supporting others (mentor / educator / supervisor) experience. Experience with using of Microsoft Office.

Knowledge

Essential

Understanding of National Service Frameworks and NICE guidance to relevant to care setting and its application across health care services. Evidence of continued relevant professional development and implementation in practice within the last 12 months. Proven ability to develop effective partnerships to achieve practical outcomes across organisational boundaries including statutory organisations, external agencies, community and voluntary organisations. Supporting others (mentor / educator / supervisor) experience.

Ability to work effectively with a variety of people at all organisational levels across different sectors including areas of social deprivation.
