Job summary

To provide high quality primary care Talking Therapy services to adults with mild to moderate mental health problems using cognitive behavioural therapy (CBT) based, self-management interventions to clients with mild to moderate anxiety and depression

To work autonomously within professional guidelines and the overall framework of team policies and procedure.

To deliver Talking Therapies services in accordance with NICE guidelines, within agreed activity levels and model, meeting required key performance indicators (KPIs).

To provide advice and consultation on client's care, to other clinicians and to non-professional carers.

To promote wellbeing and social inclusion.

To work flexibly and in accordance with service demands.

The post holder will work with people with different cultural backgrounds and ages, using interpreters when necessary and should be committed to equal opportunities

To work to the agreed number of clinical contacts per week as set by the Team Lead

Main duties of the job

Provide a high-quality low intensity psychological therapy service to individuals, carers, families & groups, within & across teams as appropriate, in line with best, evidence-based practice & trust care pathways including group therapy.

Communicate & liaise where appropriate with all other professionals involved in the individual client's treatment & on-going mental health care.

Provide regular information to client's GP &/referrer about the treatment process/outcome/initial/on-going risk assessments, making appropriate disclosure & onward necessary referrals.

Improve wellbeing through interventions. Encourage access to other agencies that provide wellbeing

initiatives & promotes social inclusion.

Make decisions regarding suitability for therapy & clinical pathways regarding issues of risk & onward referrals other specialist services as required.

Formulate & implement treatment plans with clients, normally within an agreed brief structured time frame & contracted number of sessions by offering NICE compliant interventions, NICE compliant group interventions & on-line therapy.

Exercise sound theoretical knowledge of evidence based psychological intervention & employ a range of short-term interventions meet the needs of the referred client.

Manage own clinical caseload within guidelines provided by the team lead & relevant professional guidelines & ethical framework s (PWP Code of Practice)

Job description

Job responsibilities

Accept referrals via agreed protocols within the service.

Assess and support people with a common mental health problem in the self-management of their recovery.

Undertake patient-centred interviews which identify areas where the person wishes to see change and or recovery and makes in accurate assessment of risk to self and others

Make decisions on suitability of new referrals, adhering to the departments referral protocols, refers unsuitable clients on to the relevant service or back to the referral agent as necessary or steps-up the persons treatment to high intensity psychological therapy.

Provide a range of information and support for evidence based high-volume low-intensity psychological treatments. This may include guided self-help computerised CBT, information about pharmacological treatments. This work may be face to face, telephone or via other media.

Educate and involve family members and others in treatment as necessary.

Adhere to an agreed activity contract relating to the overall number of client contacts offered, and clinical sessions carried out per week in order to minimise waiting times and ensure treatment delivery remains accessible and convenient.

Attend multi-disciplinary meetings relating to referrals or clients in treatment, where appropriate.

Complete all requirements relating to data collection within the service.

Keep coherent records of all clinical activity in line with service protocols and use these records and clinical outcome data in clinical decision making.

Work closely with other members of the team ensuring appropriate step-up and step-down arrangements are in place to maintain a stepped care approach

Assess and integrate issues surrounding work and employment into the overall therapy process.

Operate at all times from an inclusive values base which promotes recovery and recognises and respects diversity.

Qualifications

Essential

Qualification from PWP Training Course (Post Graduate Certificate or Level 3 undergraduate course) OR equivalent Experienced and qualified Graduate Mental Health Worker with relevant Primary Care experiences and competences

Knowledge/Experience

Essential

Evidence of working with people who have experienced a mental health problem Demonstrates high standards in written communication Able to write clear reports and letters Experience of working in Primary Care Services
