Job summary
This position will be to support the retired fishing community in Hull and Grimsby by travelling to local sites in our health and wellbeing mobile clinic. Health & Wellbeing Coaches support people to take proactive steps to improve the way they manage their physical health. They support people to develop their knowledge, skills and confidence in managing their health, to improve health outcomes and quality of life to support them making positive changes via coaching and through providing access to interventions such as self-management education, peer support and more.
You will work autonomously to actively promote a healthy lifestyle and support people 1-1 as well as potentially in groups to make positive changes to their health and wellbeing. Alongside this you will identify when a person's needs are beyond the scope of your role and will be required to work with patients whilst an inpatient with face support on a 1-1 basis and in groups as well as signposting and referring for ongoing support post discharge.
The successful candidate will be compassionate, reflective and self-aware and enjoy working with a wide range of people. They will have good communication skills and a firm belief that people have untapped resources within them, that can be developed by providing high quality, non-judgemental support. They will be proactive with a flexible attitude, keen to work as part of a team and committed to developing themselves in this skilled role.
Main duties of the job
Provide 1-1 and group health coaching support for people based on what's important to them Manage and prioritise a caseload, in accordance with the needs and support required by individuals. Along with a strong awareness and understanding of when it's appropriate or necessary to refer people back to other health professionals/agencies, because the person's needs are beyond the scope of the role Work as part of a multidisciplinary, multi-agency team to promote health coaching to support Personalised Care and Supported Self-Management, modelling the coaching approach in their work. Attend and contribute to team meetings and events as required Work flexibly, adapting to the needs of the service and client group while maintaining the integrity of the role Participate in regular health coaching supervision and continual learning including: Regular contact with supervisorRefresher training sessionsBuddying with peersPeer support sessions1:1 support from a practitioner with more health coaching experienceAction learning setse-learning to revisit or deepen trainingOn-going improvements to systems and processesCollect service user experience and impact of health coaching as part of the delivery of personalised care

An understanding of the biopsychosocial model of health and the social determinants of health Able to work within a biopsychosocial model, using a range of tools and techniques to enable and support people, such as agenda setting, goal setting, problem solving Ability to travel across multiple sites Commitment to continuous learning and development.
Experience
Essential
Demonstrable skills in supporting behaviour change Excellent group and one-to-one facilitation skills including conflict resolution A good understanding of physical to Foundation Degree equivalent to diploma level including health promotion, and mental health including policy/legislation
Desirable
Experience of working in a multidisciplinary team Experience of working in health and social care care/community development setting or similar Networking experience
Skills and Competencies
