Job summary

Exciting opportunities have arisen for a suitably motivated individual to work at Band 2 level with the Physiotherapy Service in Area West in the specialty of Community Falls Prevention.

The team work with people who have fallen or are at risk of falling. The team's aims are to identify and minimise falls risk, to promote wellbeing, prevent injury and unnecessary hospital admissions.

The successful applicant will provide clinical and administration support for the Community Falls Prevention Team in Area West and will need to use their own initiative to meet patient requirements and to ensure clinics / appointments are booked to ensure efficient use of clinical time.

You will be working in a challenging environment that undergoes constant changes and you will need to exercise judgment and use a combination of skills to deal with day-to-day issues.

The post holder will be expected to demonstrate a sensitive and caring attitude that is client-focused and maintain a calm and organised approach when working under pressure.

Good knowledge of IT and Windows applications, including Microsoft Office, with the ability to produce excel spreadsheets / charts etc is essential for this post.

Main duties of the job

Assist the Falls Practitioner in supporting patients as required in the delivery of strength and balance exercise programmes in the community. Support the Falls Practitioners in obtaining and recoding of patient outcome measures. Maintain accurate records in line with Health Board's Information Governance Policies. Support the Falls Practitioners with the provision of equipment and adaptations within patient homes as necessary to reduce falls risks. Process referrals by inputting patient accurate details onto the Patient Administration System, Collection and collation of patient activity figures, entering of data onto appropriate proforma as requested. Secretarial duties for all clinical staff, including word processing, and e-mailing documents, dealing with telephone messages and referring to appropriate clinician, maintaining an efficient filing and retrieval system, photocopying of correspondence as directed.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.
