About the Role Clacton PCN has a fantastic opportunity for an experienced nurse to join our team on a full-time basis for hours per week over 4 or 5 days in the role of primary care network proactive care nurse. This initial post is a 2 year fixed term contract however if the successful candidate can evidence the progress and interventions with proactive care projects we would look to make this a substantive role. As a PCN we aim to focus on improvement of quality services and over the next 2 years in particular a focus on proactively managing complex patients and reduce health inequalities in our patient population. We are excited to offer this unique opportunity for the right candidate to join our PCN team. The successful candidate will need to be forward thinking, innovative and have experience in dealing with challenging situations. There is further opportunity for training and development related to proactive care available whilst in this post. Clacton Primary Care Network (PCN) comprises of 6 local GP practices: Eastlynne Medical Centre (Employer and lead practice) Old Road Surgery North Clacton Medical group Thorpe surgery Fronks Road Harewood surgery The successful candidate may be required to work at any of the PCN member practices dependant on need and demand at time of recruitment.

The main responsibilities of the role include:

Liaise with wider MDT and support the management of both acute and chronic conditions, integrating both drug- and non-drug based treatment methods into a management plan. Providing and evaluating a range of health interventions to ensure that patients’ health and social care needs are met. Proactively identify, diagnose, and manage treatment plans for patients with or at risk of developing a long-term condition. General health and well-being promotion as well as creating individual, person-centred plans for patients with, or at risk of, a long term condition. Managing and treating long-term conditions, in line with national and local policies and the practice needs. Reviewing medication for therapeutic effectiveness, in accordance with evidence-based practice as well as national and practice protocols; working with patients to support their adherence to prescribed treatments. Prioritising health problems and intervening appropriately, to assist the patient in complex, urgent or emergency situations, including initiation of effective emergency care. Lead in the development of this new role, focussing on the needs of the practice population. Utilising population health data and working with member practices to lead them to a modern general practice model. Develop the proactive care approach for the PCN with a particular focus on frailty and prevention of deterioration of health. Utilising population health data. Leading on Patient education events for health promotion. Be a role model for PCN and practice staff, promoting proactive care and supporting new ways of working. Take a lead on Proactive care in the wider development of the PCN and local and National agenda. Assist in clinical audit processes, supporting demonstration and analysis of proactive care outcomes. Engage and evaluate peer, patient, and service evaluations. Use research and evidence-based practice to share ideas for improvements and efficiencies. Support and educate patients to adopt health promotion strategies that promote healthy lifestyles and apply principles of self-care. Please see the attached job description for further details. About the Candidate All applicants should have the following qualifications, registrations, skills and experience: A relevant nursing degree.
Current NMC registration with an active PIN. Evidence of clinical skills and experience in Holistic assessment of individuals. Compassionate manner with an empathetic approach and demonstrable kindness in all aspects of your work. The successful candidate will value the dignity of our patients, their relatives and carers, as well as their colleagues at the practice. Be able to demonstrate management of patients with complex needs Previous experience within a leadership and management role Knowledge/understanding of the Primary Care Network Direct Enhanced Service, local ICB vision/values and how primary care fits in with the NHS Long Term Plan Be able to manage own time and prioritise work effectively. Be able to work well with others and deal with the challenges of ever changing and demanding situations. Act as an autonomous practitioner whilst working within their scope of practice. Knowledge, and experience of its application, in supporting long-term conditions. Insight and understanding of current issues in nursing and the wider NHS.

About Us

Our primary care network has various clinical and non-clinical roles which consist of the following: paramedics, clinical pharmacists, physiotherapists, cancer care nurse, care coordinators, GP assistants, adult mental health clinicians, physician associates, children’s mental health worker and children’s councillor as well as social prescribers. All of these roles work towards bridging the gap between primary and secondary care as well as supporting our member practices to improve the assessment and service that we collectively provide.
