Job summary

This is an exciting opportunity to join the Paediatric Liaison Service which forms part of the Nottinghamshire Healthy Families Team Programme.

This post will support the development of the Nottinghamshire Countywide Paediatric Liaison Service, working alongside the 2 current Children and Young People's Liaison Practitioners. The post holder will predominantly support liaison with Bassetlaw Hospital however, flexibility is required to support liaison with Sherwood Forest Hospitals and Nottingham University Hospitals, as required to meet the needs of the service.

The post holder will be responsible for facilitating effective communication between the acute settings, the Healthy Families Team and primary care. They will work with acute settings to identify children & young people resident in the County who are vulnerable, have increased needs, safeguarding concerns or require ongoing services following attendance at Accident & Emergency or having been admitted to the children's wards/Neonatal Unit. They will be responsible for the prompt sharing of information between staff working with vulnerable children in acute settings and Healthy Families Teams.

The post holder will also be responsible for the co-ordination of the Care of the Next Infant (CONI) programme.

A key aspect of the role is to work flexibly to meet the needs of the service as required.

We support agile working between acute settings, community and home.

Main duties of the job

To facilitate communication, information sharing and liaison between acute hospital settings and the community. To deliver early advice and support to parents of babies admitted to the neonatal unit and support discharge planning from acute settings. In addition, to co-ordinate the Care of the Next Infant Scheme (CONI).

Job responsibilities

To facilitate communication and information sharing between acute settings, Healthy Families Teams and Primary Care.

Share relevant information with the Healthy Families Teams to pro-actively support discharge from acute settings.

To co-ordinate the Care of the Next Infant Scheme (CONI).

To share information in a timely way to Healthy Families Teams relating to serious incidents, Safeguarding concerns and Sudden Infant Death.

To work with the acute and community Safeguarding teams to highlight trends, issues and incidents related to Safeguarding.

Working with acute setting to identify risk and share relevant information with the Healthy Families Teams.

To promote effective team working, sharing knowledge and information, encouraging a supportive environment to promote good working practice and outcomes.

To be visible and accessible to acute staff, service users and families for signposting and health advice.Signpost and provide information to parents and young people on the Healthy Families Teams, communicating information in an understandable form.

To work with Nottinghamshire Safeguarding Board and organisational Safeguarding child and adult policies and procedures, being proactive in identifying concerns and promptly informing a senior member of the team.

To access and participate in Safeguarding supervision as per organisational policy.

To complete audit activities to ensure appropriate, safe and evidence base practice.

Act in accordance with the Data Protection Act and Information Governance Policy to ensure confidentiality and protection of client information.

To maintain accurate, up-to-date contemporaneous records in line with the organisation policy and NMC guidance. This includes SystmOne data entry of all activity.

To access Clinical Supervision as per organisational policy.

Qualifications

Essential

1st level Registration with current NMC registration Registered Specialist Practitioner (Health Visitor / School Nurse)

Desirable

Recognised teaching qualification (ENB 998/730/ FLIPS or equivalent). Evidence of relevant, further professional development.
