Job summary

Are youlooking to move into General Practice and develop your clinical skills? We arelooking for an additional clinical pharmacist to join our well established,exceptional Clinical Pharmacy teamworking across North Wilts Borders PCN,covering Malmesbury, Royal Wootton Bassett, Purton & Cricklade.

Thisexciting post will form part of the NHS England training & educationprogramme. Independent prescribing support and training will also be providedas part of the primary care pharmacy education pathway and you will besupported by our Senior Clinical Pharmacist and the individual surgery teams

Salarynegotiable depending on experience plus inclusion in the NHS pension scheme.

Main duties of the job

The practicepharmacist will support all practice staff, including GPs, nurses, admin staffand receptionists in providing an efficient, effective service to our patients.

Liaisingwith community and secondary care pharmacists, along with ICB medicinesmanagement team will ensure a consistent, proactive and resourceful addition tothe team.

Thepharmacist will be part of the practice multi-disciplinary team and willattend: Practice MDT meetings. Locality and community MDT meetings. Routinecare/nursing home visits including supporting GP ward rounds where appropriate

There is asenior clinical pharmacist Manager and also a GP clinical lead within each practice toenable the pharmacist to discuss patients, raise concerns and obtain advicedirectly from that practice.

Job description

Job responsibilities

Responsiblefor supporting the partners in implementing effective medicine managementwithin the practice, identifying areas for improvement, and initiating andmanaging change.

Developand manage a medicines management plan and deliver patient services asdetermined by practice policy and local and national guidance.

Developthe medicines team, including delivery of training, in order to maximisecost-effective prescribing and improve the quality of patient care.

JobRole - may include the following:

Managementof medicines at discharge from hospital

Toreconcile medicines following discharge from hospitals, intermediate care andinto care homes, including identifying and rectifying unexplained changes andworking with patients and community pharmacists to ensure patients receive themedicines they need post discharge.

Setup and manage systems to ensure continuity of medicines supply to high-riskgroups of patients (eg, those with medicine compliance aids or those in carehomes).

Riskstratification

Identificationof cohorts of patients at high risk of harm from medicines through pre-preparedpractice computer searches. This might include risks that are patient related,medicine related, or both.

Unplannedhospital admissions

Reviewthe use of medicines most commonly associated with unplanned hospitaladmissions and readmissions through audit and individual patient reviews.

Putin place changes to reduce the prescribing of these medicines to high-riskpatient groups.

Repeatprescribing

Produceand implement a practice repeat prescribing policy.

Managethe repeat prescribing reauthorisation process by reviewing patient requestsfor repeat prescriptions and reviewing medicines reaching review dates andflagging up those needing a review to the GP.

Telephoneand patient facing

medicinessupport

Providea telephone help line for patients with questions, queries and concerns abouttheir medicines.

Holdclinics for patients requiring face-to-face medicines use reviews (MURs) ie,advise about medicines and adherence support.

Medicationreview

Undertakeclinical medication reviews with patients and produce recommendations for theGP on prescribing and monitoring.

Carehome medication reviews

Undertakeclinical medication reviews with patients and produce recommendations for theGP on prescribing and monitoring.

Workwith care home staff to improve safety of medicines ordering andadministration.

Domiciliaryclinical medication review

Undertakeclinical medication reviews with patients and produce recommendations for theGP on prescribing and monitoring.

Long-termcondition clinics

Seepatients with single medical problems where medicine optimisation is required(eg, COPD, asthma).

Makerecommendations to GPs for medicine improvements.

Servicedevelopment

Contributepharmaceutical advice for the development and implementation of new servicesthat have medicinal components (eg, advise on treatment pathways and patientinformation leaflets).

CareQuality Commission

Workwith the practice manager and GPs to ensure the practice is compliant with CQCstandards where medicines are involved.

Undertakerisk assessment and management and ensure compliance with medicineslegislation.

Publichealth

Tocontribute to public health campaigns, including flu vaccinations and adultimmunisation programmes.

Costsaving programmes

Reviewpotential changes to medicines (switches) designed to save on medicine costswhere a medicine or product with lower acquisition cost is now available.

Medicineinformation to practice staff and patients

Answersall medicine-related enquiries from GPs, other practice staff and patients withqueries about medicines.

Informationmanagement

Analyse,interpret and present medicines data to highlight issues and risks to supportdecision making.

Medicinesquality improvement

Undertakesimple audits of prescribing in areas directed by the GPs, feedback the resultsand implement changes in conjunction with the practice team.

Training

Provideeducation and training to primary healthcare team on therapeutics and medicinesoptimisation.

Implementationof local and national guidelines and formulary recommendations

Monitorpractice prescribing against the local health economys RAG list and makerecommendations to GPs for medicines that should be prescribed by hospitaldoctors (red drugs) or subject to shared care (amber drugs).

Assistpractices in setting and maintaining a practice formulary that is hosted on thepractices computer system.

Auditingpractices compliance against NICE technology assessment guidance.

Providenewsletters or bulletins on important prescribing messages.

Medicinessafety

Implementchanges to medicines that result from MHRA alerts, product withdrawal and otherlocal and national guidance.
