The virtual ward is an innovative service which a wide geographical area across Staffordshire and provides acute hospital based care within the community setting to enable patients to receive care in their own homes. We are looking for dynamic and forward thinking individuals with an acute care background to provide education and leadership within our team. The ideal candidate will have proven health assessment skills and ideally be an independent non-medical prescriber or working towards this qualification.

The post holder wil manage the Acute Virtual Ward ensuring beds available against agreed pathways to deliver a 7 day 8-10pm service on a day to day basis. A list of main tasks and duties are outlined below but are not exhaustive due to the evolving nature of this unique role and service.

We currently provide a 7 day service and there is opportunity for flexible working.

The role will be community based, therefore the candidate must be able to hold a UK driving licence and must be able to commute.

Main duties of the job

Demonstrates clinical expertise and leadership qualities on a daily basis in order to support the overall running of the service whilst supporting the Virtual Ward Matrons. Confidently lead on ward rounds, daily huddles and multidisciplinary team meetings, establishing and maintaining effective working relationships Able to formulate appropriate management plans for patients, whilst continuously revaluating initial findings and clinical response. Able to assess new referrals and establish the level of support the individual may require from the service.

Is able to undertake a physical examination assessment as indicated by the patient's condition, formulating, implementing and evaluating treatment plans for individuals, families and carers

Prescribing and deprescribing of medications for therapeutic effectiveness appropriate to patient need and in accordance with best/evidence based practice and national and local protocols and within the role's scope of practice and legal framework Is able to take the lead in complex case management, providing support and advice to others.
