Job summary

An exciting opportunity has arisen for a Band 4 Patient Pathway Coordinator within Nuclear Medicine, Thyroid/NET team.

This is an excellent opportunity for those looking to develop their experience and skills whilst working in a fast-paced environment focusing on outpatient pathways. We are looking for a highly organised person who is able to prioritise a busy workload in a demanding environment and perform well under pressure.

The successful candidate will have a good understanding of clinical systems, strong communication skills, attention to details and be proactive to achieve desired outcomes.

Main duties of the job

The post holder will provide a range of administrative support to the multi-disciplinary clinical team, whilst working very closely with clinical teams.

The Patient Pathway Coordinator will lead on the tracking of pathways of patients, process referrals, record data on internal database/s within agreed timeframes, manage any data requests, collate clinical data onto databases and work closely with the MDT.

By working closely with a wide range of clinical and managerial staff, and providing high standard of administrative support, the post holder will ensure patients receive treatment as soon as possible and ensure national targets are met.

Job description

Job responsibilities

The post holder is responsible for providing a high standard of professional and patient focused service and will support the day-to-day operations of the department.

Booking of Therapies using EPIC system and sending out paperwork and information to patients. Requesting blood requests from GPs and other hospitals Scanning paperwork from GPs and other hospitals Making sure outcomes are recorded correctly on EPIC Looking at EPIC for clinic outcome letters Adding data to Thyroid/NET spreadsheets that Physics, CNSs and Consultants have access too. Attending the MDM meeting with Consultants, CNSs Physics etc. Covering the Thyroid clinic reception when staff on leave, sickness or training. Dealing with other request CNSs may wish you to action or Consultants may wish you to action. Consent Scanned and emailing of discharge instructions Checking for new referrals on account. Where required, requesting imaging be loaded up by PACs team. Reviewing and placing outcomes for the following report Outpatient RTT outcome (sent weekly) Investigate and fact find reports to feedback and answer in a timely manner back to managers.
