Both Basingstoke and Winchester Older Persons Mental Health Community Teams have an exciting opportunity for a Band 3, Health Care Support Worker to join - both part time and full time can be considered.

The teams are highly supportive and innovative, aspiring to provide a responsive, safe, effective, caring service that is well led.

The Older Persons Mental Health Service is looking for motivated health care support workers to provide and deliver skilled interventions to older people with mental health needs in their home environments.

The role involves providing post diagnostic support to those diagnosed with dementia and supporting individuals with other mental health diagnosis to feel included in the community to continue to manage their own wellbeing and independence at home, as well as providing support and signposting to families and their carers.

To be a successful member of our teams you will need to be motivated, passionate and have a genuine interest in working with older people. You will need to be able to demonstrate a real commitment to providing high quality and compassionate care to patients and their families and be a supportive team member.

Main duties of the job

*To support patients with dementia and their carers in the home environment through post diagnostic follow-up, signposting, providing carer's support and education.

*To support people with functional mental health problems and their carers in the home environment through challenging times by working alongside the wider team.

*To support the team, including nurses, doctors, psychologists through flexible and proactive working.

*To support the duty nurse with answering incoming calls, referring, and signposting to agencies.

*To be competent in reviewing mental state, identifying risks and safeguarding concerns.

*To effectively communicate with the wider team.

*To accurately record service user information.

*To manage a caseload.

*To develop a care plan in collaboration with the patient and carer of goals to be achieved during HCSW provided care.

*To undertake baseline physical observations.

The transformation will happen in stages; Hampshire CAMHS, part of Sussex Partnership NHS Foundation Trust joined Southern Health in February 2024, with Isle of Wight NHS Trust mental health and community services transitioning across in May 2024. The final combination of Southern Health and Solent NHS Trust services is expected in late 2024, when our new organisation, to be known as Hampshire and Isle of Wight Healthcare NHS Foundation Trust, will be formed.

Our new Trust will continue to cater to the unique needs of different communities, making healthcare across the county more accessible, as well as offering staff more opportunities for career development, training and partnership working.
