Job summary
This post is a 12 month fixed term contract to provide maternity cover. The post holder will work within their clinical competencies as part of a multi-disciplinary team to provide expertise in clinical medicines management, provide face to face structured medication reviews, manage long term conditions, management of medicines on transfer of care and systems for safer prescribing, manage repeat prescription authorisations and re authorisation, acute prescription request, while addressing both the public health and social care needs of patients. This role is pivotal to improving the quality of care and operational efficiencies so requires motivation and passion to deliver an excellent service within general practice.

Complete the CPPE Development Pathway whilst in role if not already done so. Perform structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy and care home residents Be involved in the management of long-term conditions Embrace service development to grow their capabilities and the clinical pharmacy teamThe ideal candidate will be a Pharmacist with clinical experience and being an independent prescriber is desirable. We are looking for someone who is committed to improving patient care and is passionate about clinical pharmacy. The candidate must be able to work effectively as part of the practice team and be comfortable in encouraging new ways of working. Whilst prior experience in General Practice would be an advantage, pharmacists from other sectors with transferable skills are very welcome.

Job responsibilities
The post holder will be expected to:
Perform structured medication reviews (SMRs) to proactively manage patients with complex polypharmacy and care home residents
Be involved in the management of long-term conditions
Embrace service development to grow the capabilities of the clinical pharmacy team
Help to deliver and maintain work streams within the prescribing scheme that is locally set.
Work closely with GP's on-site and other teams to respond to requests and queries relating to medication and prescribing.
The ideal candidate will be a Pharmacist with significant clinical and quality improvement experience and be an Independent Prescriber. We are looking for an individual with leadership qualities, who is committed to improving patient care and is passionate about clinical pharmacy in Primary Care.
The candidate must be able to work effectively as part of the practice team and possess the skills and resilience to manage rapid change. Although prior experience in General Practice would be an advantage, pharmacists from other sectors with significant transferable skills will be considered for the role.

Qualifications
Essential
GPhC registered pharmacist and must hold a a GPhC independent prescribing qualification. Have a minimum of 2 years working as a pharmacist demonstrated within a practice portfolio. Experience in managing pharmacy services in primary care. Experience and an awareness of common acute and chronic conditions that are likely to be seen in general practice. Ability to communicate complex and sensitive information effectively with people at all levels by telephone, email and face to face
Desirable
Membership of the Royal Pharmaceutical Society. Working towards faculty membership of the Royal Pharmaceutical Society Medicines Management Qualification MUR and repeat dispensing certification

Minimum of two years working as a pharmacist demonstrated within a practice portfolio An appreciation of the nature of GPs and general practice.
