Job Summary

Key Responsibilities
  • Work closely with General Practitioners, practice staff, community pharmacists, care home staff, and primary healthcare teams to develop and apply high-quality, safe, cost-effective, and evidence-based prescribing practices.
  • Provide individualized clinical care to patients through clinics, improving the safety and effectiveness of their medicines, and prescribing where appropriate.
  • Have a sound clinical knowledge, good IT skills, excellent communication and interpersonal skills, and be able to work autonomously as well as part of a team.
  • Work on joint projects and initiatives within the Health and Social Care Partnership, promoting awareness of prescribing management issues.
Requirements
  • Masters Degree in Pharmacy or equivalent
  • Registrant with the General Pharmaceutical Council
  • Independent prescriber or working towards
  • Minimum of 2 years' post-registration experience
