Job summary
An exciting opportunity has arisen for a Band 7 Senior Nurse to lead in our Community Coordination Centre.
The post-holder will work as an autonomous clinical practitioner and lead a multidisciplinary team to effectively run and maintain our South East Essex 'Community Coordination Centre' (CCC), also commonly known as a 'transfer of care' hub.
The post holder will provide clinical leadership and oversight to the CCC, delivering specialist advice in respect to community services and be key interface between patient/professional and the community service offer. The post-holder will hold close working relationships with a range of partners and will engage in local system transformation relating to integrated streamlined access to community services.
The CCC operates to provide streamlined urgent and same day access to community services 7 days a week, through a high quality call-handling (using dedicated call-handling telephony infrastructure) function, expert triage function, and expert patient management function.
Main duties of the job
The post holder will be responsible for ensuring that all patient activity processed though the CCC is effectively managed, in a timely manner, to ensure patient safety and the highest possible quality and standard of care.
The post-holder will be responsible for the development of the CCC team, including performance management, appraisals, recruitment and retention of staff, monitoring sickness and absence and providing education and training as required.

Job description
Job responsibilities
To communicate complex and sensitive information in an understandable form to clients and carers who may be distressed or have difficulty coming to terms with a diagnosis, a condition/symptom, a situation and its implications.
Contribute effectively, working as part of the Integrated Health and Social Care Team providing nursing advice which contributes to the care for all patients, collaborating with statutory and non-statutory organisations.
To use appropriate methods to facilitate effective communication with clients and carers who may experience communication difficulties those who have mental health needs, learning disabilities cognitive or sensory impairment or those who require interpreters.
To develop effective communication mechanisms between patients; relatives; carers and multidisciplinary team members. Initial response to all urgent clinical issues. Accountability for delivery of service within Community Coordination Centre
To be able to hold positive relationships in highly challenging and emotive atmosphere.
To communicate sensitive/highly sensitive, confidential information concerning patients/clients with empathy.
To develop and maintain strong positive professional relationships with a range of multi-disciplinary professions and across system providers ( Social Care, Surgeries, PCN, Community Collaborative, Vol Sector).
To actively promote community nursing and integrated working internally and externally.
To be mentor to pre-registration students and new staff.
Use advanced skills and specialist knowledge to undertake virtual assessment of health and social care needs. This will involve establishing both the physical and psycho-social needs of the patient to inform the development of an appropriate response and care.
Support and make direct referral of patients for appropriate community service assessment and procedures.
Plan interventions in light of patients wishes and goals of care, facilitating discussion with the family and significant others. Negotiate and agree with patient, carers and other professionals individual roles and responsibilities.
Develop evidence-based treatment plans based on sound clinical decision-making.
Ensure patients with palliative care needs receive the highest standards of timely response, care and support, respecting their wishes and aligning care to the Gold Standards Framework, Preferred Place of Care and Advanced Care Planning.
In partnership with patients and carers, facilitate a self-management approach to achieve optimum levels of self-care.
Work jointly with Adult social care colleagues in assessing and implementing complex care interventions in a pro-active manner.
Work with PCN GP practices providing support and management of those that are severely frail to avoid unnecessary hospital admissions.
