Job summary

We are a forward thinking training practice looking for a Paramedic Practitioner or Nurse Practitioner to join our evolving clinical team.

We are looking for a dynamic team player who would be primarily managing on the day demand for acute presentations alongside our current paramedic.

Snodland Medical Practice has purpose built modern premises, with ample parking. We are a practice of 12,000 patients, 7 GP's, 1 Paramedic practitioner, 3 nurses, 2 Health Care assistants and 1 phlebotomist. We currently host 3 GP trainees.

Main duties of the job

We are looking for a person who would manage on the day demand for acute illness.

Formal ACP masters qualification is not essential but experience in assessing minor illness cases and formulating management plans would be required. A primary enhanced clinical skills qualification is desirable.

We have in-house experience in mentoring through Enhanced Clinical Skills, Prescribing and ACP Masters qualifications. We actively support in-house development and career progression. We have regular clinical meetings to discuss cases, role development and provide clinical teaching.

About us

Our practice has good QoF attainment and was rated "Good" in our recent CQC inspection.

Our GP's have special interests in education, cardiology, minor surgery, frailty, ophthalmology and mental health and we have in-house capability or starting diabetes injectables.

Job responsibilities

Paramedic Practitioner/Nurse Practitioner

The post-holder will besupported through indirect supervision to be responsible for assessing, anddiagnosing, with progression to interpreting investigations, and treatingpatients within Primary Care, developing a scope of competence. They willmonitor and assist with improving standards of care utilising clinicalexpertise and experience. They will teach and support colleagues whereappropriate, providing professional clinical leadership. Where appropriate theywill lead and/or participate in initiatives to improve standards of patientcare.

The post-holder is servicedriven and will develop themselves using the Department of HealthGuidance across the four domains of Advanced Practice, these are:

JOBPURPOSE

Thepost holder will be an experienced Paramedic qualified to undergraduate level.They will:

Be able todemonstrate safe, clinical decision-making by autonomously assessing patientspresenting to Primary Care including initial history taking, clinicalassessment and diagnosis.

Following initialin-house training, they will be able to advise on treatment, andevaluate care with suggestions of (or give prescription if a prescribingqualification is held) any medication required.

Demonstrate criticalthinking in clinical decision-making. They will work collaborativelywith the practice team to meet the needs of patients andwill play a key role in service delivery and adherence to policy andprocedures.

Provide and assuresafe care to patients and maintain a safe environment for allaccording to local and national standards.

Facilitate aneffective workplace culture with the practice and in cross-organisation workingif needed.

Sustainperson-centred, safe and effective care through self-awareness, leadership,active learning and development.

Constantly seek to furtherskills and knowledge to enhance patient care, meet the changing needs of thepractice.

3. KEY RESULT AREAS/MAIN DUTIES ANDRESPONSIBILITIES:

Practice

1. Workautonomously (within defined patient pathways of care) to assess,examine, investigate, diagnose and treat patients with a wide range ofphysical, social, and psychological needs and presenting complaints.

2. Undertakeconsultations in an appropriate format (face-to-face, telephone, virtual),taking into account the clinical presentation and patient preference.

3. Undertaketelephone triage work and telephone advice calls.

4. Undertakehome visits as assessed by the GP or Advanced Nurse practitioner.

5. Diagnoseand manage both acute and chronic conditions, integrating both drug- andnon-drug treatments into a management plan.

6. Undertakeappropriate investigations and treatments utilising evidence-based practice/clinicalguidelines applying skill, knowledge and clinical judgement tomeet the patient needs.

7. Interpret information using knowledge and judgement to provide appropriate advice when evaluating all aspects of the patient care andtreatment plan, according to their holistic needs.

8. Discusswith registered GP if needed, and to make referrals to otherprofessionals in primary and secondary care.

9. Prioritisehealth problems and intervene appropriately to assist the patient in complex,urgent or emergency situations, including initiation of effective emergencycare.

10.Supportpatients to adopt healthy lifestylesand apply principles of self-care.

11.Proactivelyidentify, diagnose, and manage treatment plans for patients at risk ofdeveloping long-term conditions (as appropriate).

12. Be aware of own limitationsand refer to other practitioners when needs and risks are beyondtheir own scope of practice.

13. Obtain consent whenundertaking assessments. When discussing implementation of investigation andtreatment plans, give alternatives, if appropriate to facilitate patientchoice.

14. Evaluate the effectivenessof interventions and treatment using evidence-based practice, whilst usingknowledge and skills to assess further patient needs.

15. According to currentlegislation and practice policy undertake independent, non-medical prescribingif relevant.

16. Maintain accurate patient records, both written and electronic, in accordance with localand national professional policies and guidelines.

17. Ensure that all practicewithin the clinical area complies with practice policies and procedures.

18.Practicein accordance with the professional and legal framework for advancedclinical practice and that of their registered professional body.

19. Where issues of clinicalcompetence are identified, work with their supervisor to address them.

20. Contribute to the resolution, investigation and follow action of any informal or formalcomplaints.

21. To maintain a professional portfolio of evidence for personal& professional development.
