Job summary

The PCN Cancer Care Coordinator willsupport GP practices within the Primary Care Network, working withinprofessional and clinical boundaries as part of an establishedmulti-disciplinary team to deliver timely and personalised care for patients,and deliver key objectives of the Primary Care Network DES. This post willparticularly be supporting the early cancer diagnosis and cancer care qualityimprovement work by supporting practices to improve their processes, achievetheir targets and working with patients to help them ensure they have the rightsupport at each stage of their journey.

Main duties of the job

Support practices to delivertheir quality improvement plans for early cancer diagnosis.

Develop and embed systemsacross the network to improve cancer screening uptake, liaising with externalagencies as appropriate

Utilise population healthintelligence to proactively identify and work with patients newly diagnosedwith cancer and on the cancer register to deliver personalised care;

Ensure patients receive aCancer Care review in line with national defined timescales and targets.

Support patients to utilisedecision aids in preparation for a shared decision-making conversation;

Holistically bring together allof a persons identified care and support needs, and explore options to meetthese within a single personalized care and support plan (PCSP), in line withPCSP best practice, based on what matters to the person;

Help people to manage theirneeds through answering queries, making and managing appointments, and ensuringthat people have good quality written or verbal information to help them makechoices about their care;

Support people to take uptraining and employment, and to access appropriate benefits where eligible;

Job description

Job responsibilities

Assist people to accessself-management education courses, peer support or interventions that supportthem in their health and wellbeing and increase their activation level;

Explore and assist people toaccess personal health budgets where appropriate;

Provide coordination andnavigation for people and their carers across health and care services, workingclosely with social prescribing link workers, health and wellbeing coaches, andother primary care professionals;

Support the coordination anddelivery of MDTs within the PCN.

Work with the GPs and otherprimary care professionals within the PCN to identify and manage a caseload ofpatients, and where required and as appropriate, refer people back to otherhealth professionals within the PCN;

Raise awareness within the PCNof shared-decision making and decision support tools;

Raise awareness of how toidentify patients who may benefit from shared decision making and support PCNstaff and patients to be more prepared to have shared decision-makingconversations.

Safeguard patients by ensuringorganisations and groups to whom its Care Coordinator directs patients havebasic safeguarding processes in place for vulnerable individuals and provideopportunities for the patient to develop friendships and a sense of belonging,as well as to build knowledge, skills and confidence.

Duties will vary from time totime under the direction of the Primary Care Manager and Practice Managerdependent on current and evolving PCN workload and staffing levels.
