Job summary

Rural Primary Care Network (PCN) is looking to recruit an Advanced Clinical/Nurse Practitioner to join our PCNs Multi-Disciplinary Team (MDT). PCN team have built excellent working relationships between themselves and colleagues across the PCN, practices and colleagues from other PCNs. This is an exciting time to join Primary Care and shape the future of patient care at the first points of contact. We welcome applications from ACPs with the following specialisms, Nursing, Paramedic and Clinical Pharmacist.

PCN's focus would be delivering PCN DES and improve capacity and access plan.

Please note this is not an Agenda for Change Contract, however we are able to offer the NHS Pension Scheme to the successful candidate(s).

Main duties of the job

First point of contact and direct access for patients to assess and manage patients with undifferentiated case load, patents of all ages discussing and planning investigations, treating, and referring patients as appropriate with undifferentiated and undiagnosed problems seeking using various triage models, online triage, face to face consultations, and home visits. Work with the PCN digital team to enable various IT and digital solutions evaluation and implementation.

Ability to make professionally autonomous decisions based on knowledge and skills, including skills such as physical examination, applied pharmacology and evidence-based prescribing.

Have awareness of the Network Contract Directed Enhanced Service (Network DES) and work to achieve IIF targets relevant to the role. Explore opportunities to work across PCN and enable new ways of working.

Maintain quality patient records and use a database to store details of referrals and care pathways for specific patients.

Assist with the delivery of training to a range of people within or outside the profession in consultation with the GPs, Nursing team and other members of the primary health care team. Regular evaluation and update of PCN protocols and guidelines.

Develop effective working relationship with PCN member practices.

Develop and maintain productive relationships with key stakeholders; GPs, consultants, district nurses, social services, Trusts, voluntary sector etc.

Job responsibilities

To be able to work autonomously yet deliver results within a very well-established team of clinical professionals.

Develop one's own knowledge and practice to meet objectives/changes in service/practices.

To undertake training and education as appropriate to continuously advance scope of professional practice.

Contribute to research, audit, teaching and professional development of colleagues, PCN project ;frailty, proractive care, Enhanced Health for Care Home.

Contribute to the recruitment and selection of team members when applicable.

Manage the implementation of change in work activity and service.

Undertake research and audit as required.

Access clinical supervision.

Support with training and supervision of less qualified staff.

Complete the relevant training to provide multi-professional clinical practice and CPD supervision to other roles

appropriate provide definitive treatment (including prescribingmedications following policy, patient group directives, NICE (national)and local clinical guidelines and local care pathways) or make necessaryreferrals to other members of the primary care team;b. manage undifferentiated undiagnosed condition and identify red flagsand underlying serious pathology and take appropriate action;c. use complex decision making to inform the diagnosis, investigation,complete management of episodes of care within a broad scope ofpractice;d. actively take a personalised care approach and population centred careapproach to enable shared decision making with the presenting person;ande. complete the relevant training in order to provide multi-professionalclinical practice and CPD supervision to other roles within primary care,for example first contact practitioners and the personalised care roles.

Person Specification

Qualifications

Essential

Degree Level Education, NMC registered; MSc in Advanced Practice (accredited by Centre for Advancing Practice); (Have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice); Independent prescriber; CPD Portfolio to demonstrate learning and development For advanced practice pharmacists only, has qualified from an approved 18-month training pathway or equivalent and qualified as an independent prescriber; and d. for Advanced Clinical Practitioner Nurses only, have either graduated from a Centre for Advancing Practice accredited MSc advanced practice programme or completed the Centres ePortfolio (supported) Route; both pathways enable eligibility for an Advanced digital badge issued by the Centre, demonstrating recognised educational and experiential preparation in advanced practice
