Specialist Nurse Chronic Pain Management Band 7

The post holder will be working within the Chronic Painspecialty. Liaising closely and maintaining good working relationships withTheatres, Physiotherapy department, X-ray department, Psychology department,Out - patient department, health records and referring General Practitioners, unit, community nursing and mental health trust. The post holder will workas a highly specialized practitioner autonomously within the nurse led clinicand multidisciplinary pain setting assessing, planning, delivering, co-coordinating, and evaluating care for patient and carers, providing specialistadvise and counselling.

External

To liaise closely and maintain goodworking relationships with referring General Practitioners and unit, andcommunity nursing staff.

To maintain close links with mentalhealth trust.

Main duties of the job

Principle responsibilities include developing an innovatedapproach to pain management. Implementing safe and effective practices for thecare and treatment of patients suffering with chronic pain.

To work as a highly specialist practitioner autonomouslywithin the multidisciplinary pain clinic, planning, delivering, co-coordinatingand evaluating care for patients with chronic pain syndromes.

To work as a highly specialist practitioner autonomouslydeveloping nurse led clinics and providing telephone support / help-line toreview follow-up patients and to see new referral for trials of modulatedelectro analgesia devises and acupuncture course.

To prioritise, organise and manage clinical workload andpersonal clinical workload efficiently. Co-ordinate therapy and individualtreatment programmes for a case load of patients with chronic pain syndromesand psychosocial problems, maintaining records as an autonomous practitioner. To organise and manage the Multidisciplinarypain clinic along side the pain consultant, specialist physiotherapist, andclinical psychologist.

The post holder will be part of the painmanagement programme team and will help support the patients in managing theirpain outside of an acute setting and be involved in the triaging for theprogramme. It will involve group work with patients and support from the team.

Key Relationships

To maintain effective communication with members of themultidisciplinary pain team, anaesthetic services and palliative care,orthopaedic and pain teams across the trust.

To liaise closely and maintain good working relationshipswith Theatres, Physiotherapy department, X-ray department, Psychologydepartment, Out - patient department, health records and support staff.

To communicate effectively as a mentor for newly appointedspecialist nurse working within the chronic pain specialty, promotingprofessionalism and undertake clinical work to maintain their professionalcompetence.

To maintain close links and build good working relationshipswith Medical and nursing staff and other health professionals.

To liaise closely and maintain good working relationshipswith referring General Practitioners and unit, and community nursingstaff.

To maintain close links with mental health trust.

Budgetary management Manage budget allocation in conjunction withspecialty manager demonstrating budgetary awareness, and the efficient andeffective utilization of resources.

Countywide working Excellentcommunication links with chronic / acute pain team across the county. Tocommunicate effectively as a mentor for newly appointed specialist nurseworking within the chronic pain specialty.

Takes direct referral from consultants. To work as a highlyspecialist practitioner autonomously taking direct referrals from consultantsfor new trials of modulated electro analgesia devises.

Takes direct referral from GPs. To work as a highlyspecialist nurse autonomously taking referrals direct from generalpractitioners for new trials of modulated electro analgesia and follow-up.

Undertakes roles formally/normally undertaken by medicalstaff Provides specialist advice,contributes to diagnosis through highly specialized subjective assessment andprovides a planned episode of treatment / pain management.

Providing electro modulated analgesia treatments.

Works autonomously of medical staff no directsupervision and independent decision-making. To work as a highly specialistpractitioner autonomously providing nurse led clinics reviewing follow-uppatients decision making and implementing treatment of electro analgesia,acupuncture, medication advice, addingpatients directly to consultants waiting list for invasive treatments andproviding specialist advice and counselling to patients and carers, also viatelephone help line.

Communication & Relationship skills Receives multidimensional complex problemsinvolving pain management, emotional, psychosocial issues with patients /carers suffering with chronic pain syndromes requiring lengthy consecutiveconsultations counselling, reassurance, frequently requiring negotiation,motivation, and education promoting patient interaction within an empathicrelationship.

Analytical & judgmental skills. To use highly advanced judgmental skills and a comprehensiveknowledge of pain management treatments and interventions to formulate aspecialist individual treatment plan based on evidence based practice,constantly evaluate its effectiveness and modify as it is necessary. l

Planning & organizational skills. On daily basis priorities, organize and manage own clinical workloadeffectively and efficiently.

Co-ordinate activities with other departments regardingmultidisciplinary pain clinic, theatres, x-ray, physiotherapy and clinicalpsychology department.

Actively organize and contribute to departmentalmultidisciplinary pain meetings / case conferences.

To play active part in the future development ofmultidisciplinary pain clinic.

Physical skills. Thepost requires physical skills, which have been obtained through training acupuncture, electro nerve modulation.

Preparation of specialized equipment Radio frequencylessoning machine, pressurized tourniquet.

Responsibility for patient / client care. To work as a highly specialist practitionerautonomously within the nurse led clinic and multidisciplinary pain settingassessing, planning, delivering, co- coordinating, and evaluating care forpatient and carers, providing specialist advise and counselling.

Responsibility for policy/service development ,Service development, aspires to developprotocols for nurse led pain relieving treatments and pain management withinthe service.

Provides evidence based patient information leaflets.

Provides guidelines for the care of patients pre and postinvasive treatments within day case setting.

Following completed documentation / policies aspires todevelop and participate in the setting up and running of a countywide painmanagement programme.

Responsibility for financial & physical resources. Responsible for ensuring safe use of allmechanical / electrical equipment used in the care and treatment of chronicpain patients.

Maintain stock control of specialized equipment andresources, responsible for selecting suppliers demonstrating budgetaryawareness.

Responsibility for human resources. Responsible for the teaching and education of all grades ofstaff to provide supervision and support as necessary.

Act as a resource for other health professionals on chronicpain conditions.

To take an active part in the in-service training of themultidisciplinary pain clinic team as a recipient and when appropriate as theprovider.

To be involved in recruitment and selection of nursing staffas required.

Responsibility for information resources. To use appropriateInformation Technology Skills to facilitate patient management.

To maintain comprehensive accurate assessment and treatmentrecords in medical notes as an autonomous practitioner, also in the form ofletters to referring agent GPs.

Responsibility for research & development. Involved in continuing Pain AuditCollection System.

Regularly undertakes formal pain clinic patient satisfactionaudit.

Works as a highly specialist nurse autonomously seeingpatients for trial treatments of electro analgesic devises.

Freedom to act. To workas a highly specialist practitioner autonomously providing nurse led clinicsreviewing follow up patients decision making and implementing treatments ofelectro analgesia, acupuncture, medication advise, pain management strategiesand counselling. Adding patients directly to Consultants waiting list forinvasive treatments and providing specialist advise and counselling, viatelephone help line and multidisciplinary pain clinic.

Works as a highly specialist practitioner alongside clinicalphysiotherapist seeing new patient referrals in the absence of the pain clinicconsultant.

Physical effort. Frequentmanual handling activities include positioning patients for treatments, pushingwheel chairs, trolleys and items of equipment and assisting with thetransferring of patients.

Mental effort. Thepost requires constant high mental effort. There is constant need for prolongedperiods of high concentration during consecutive subjective assessments andnurse consultations.

Emotional effort. There is constant need for frequentprolonged periods of consecutive counselling of very emotionally distressedpatients / carers.

Working conditions. There is frequent close contact withpatients, which exposes the professional to infections and possibility ofexposure to uncontained body fluids and needle piercing injury, verbalaggression from patient/carers.

Essential

Current registration on relevant part of NMC register RN . Teaching & Assessing. Or equivalent course. Principles of Pain Management or equivalent course . 1st line Management course. Evidence of in-depth knowledge of chronic pain and pain management. Ability to work as an autonomous practitioner in a challenging & dynamic way.

Desirable

Looking towards relevant degree level study. Counselling course. Acupuncture course. Nurse Prescriber or willingness to do training to achieve this. Experience of supporting a Pain Management Programme [PMP]
