Job summary

BrownlowHealth is a large at-scale General Practice, based over four sites in CentralLiverpool. We serve 80,000 patients, and are part of Central Liverpool PrimaryCare Network, which incorporates 9 practices, covering 110,000 patientscity-wide. As well as providing traditional general practice to a diverseinner-city population across a wide demographic, we are also a large Universitypractice and serve several Inclusion Health groups. This includes but is notlimited to serving both homeless and Refugee and Asylum populations in Central Liverpooland supporting patients with issues related to substance misuse. We have alarge multidisciplinary work force, including a pharmacy team consisting of apharmacy manager, pharmacists, pharmacy techs and prescribing clerks. Ourvalues include being quality driven, accountable and readily adaptable. We aimto provide efficient, compassionate care to our patients, and recognise theimportance of the team in delivering this.

We are looking for a senior pharmacist to join ourpharmacy team to work alongside another two senior pharmacists to lead a largeteam of pharmacists, technicians and prescribing clerks.

Main duties of the job

Job responsibilities include patient facing clinical medication reviews, medicine information to practice staff and patients, repeat prescribing, medicines quality improvement, medicines safety.

Job responsibilities

Patient facing Clinical Medication Review:

Undertake by means of face-to-face clinics, domiciliary visits, telephone and virtual medication review.

Perform Structured Medication reviews for patients on both single medications, and polypharmacy patients.

Review the indication for each medicine, compliance, monitoring arrangements and ensure patients get optimal benefit from their medication

Make appropriate recommendations as needed

Review and completion of shared care agreements

Partake in an on-call pharmacy rota in hours, addressing on the day phone calls, queries and providing medicines support to the duty clinical team.

Undertake changes to patients medication from hospital discharge and clinic letters.

Undertake clinical drug reconciliation for new patients registered with the practice

Provide a comprehensive prescribing service to care homes in conjunction with care home MDT and named staff for each care home

To provide services linked to prescribing that improve patient access

Medicine information to practice staff and patients:

Handle relevant medicine-related enquiries from GPs, other practice staff, other healthcare teams ( community pharmacy) and patients with queries about medicines.

Repeat prescribing:

Assist GP practices with the implementation and delivery of their repeat prescribing policy

Service development:

Contribute pharmaceutical advice for the development and implementation of new services linked to medicines management ( advice on treatment pathways and patient information leaflets).

Mandatory registration with General Pharmaceutical Council Masters degree in pharmacy (MPharm) (or equivalent Specialist knowledge acquired through post- graduate diploma level or equivalent training/experience Independent Prescriber

Membership of the Royal Pharmaceutical Society A member of or working towards Faculty membership of the Royal Pharmaceutical Society

Minimum of 4 years post- qualification experience 2 years experience in primary care An appreciation of the nature of general practice An appreciation of the nature of primary care prescribing, concepts of rational prescribing and strategies for improving prescribing Excellent interpersonal, influencing and negotiating skills Excellent written and verbal communication skills Is able to plan, manage, monitor, advise and review general medicine optimisation issues in core areas for long term conditions Good IT skills, including previous experience of EMIS web and EPS systems Able to obtain and analyse complex technical information Recognises priorities when problem solving and identifies deviations from the normal pattern and is able to refer to seniors or GPs when appropriate Able to work under pressure and to meet deadlines Work effectively independently and as a team member Demonstrates accountability for delivering professional expertise and direct service provision
