You will be part of an innovative multidisciplinaryteam working across the Bridgedale PCN to support and assist the GP practiceswithin the PCN.

We are looking for dynamic and pro-active GPAssistant to assist in improving patient outcomes and support the primary careworkforce in general practice.

Riverside Medical Centre

The Ryan Medical Centre

New Longton Surgery

Lostock Hall Medical Centre

Other associated PCN sites

Main duties of the job

OurPCN is looking for a GP Assistant to join our growing primary care workforce.The successful candidates will work across the patch, within the PCN Practices,Community Teams and across the Multi-Disciplinary Team (MDT).

AGP Assistant will deal with patients and, if appropriate, their carer, beforeor after the patients consultation with a clinician or other healthcareprofessional.

TheGP Assistants role requires them to be able to work with, and understand theroles of, a variety of different people working in the practice and across thePCN including doctors, nurses, healthcare assistants, social prescribing linkworkers, physiotherapists, paramedics, health and wellbeing coaches, andpharmacy technicians.

AGPA will support the GP(s) smooth running of clinics by performing the moreroutine administration and clinical tasks on behalf of the GP freeing up theirtime to focus on the patient.

Job responsibilities

CORERESPONSIBILITIES

Sorting all clinical post and prioritising for the GPin terms of actions.

Signposting some post to others such as clinicalpharmacist etc.

Running searches to identify patients for specific groups.

Sending bulk text messages to invite patients to groups.

Updating patient records with codes after attending groups.

Follow up information sent to patients via email.

Room set-up

Taking routine measurements such as height, weight, BP as/when

required.

Arranging appointments, referrals, tests and follow upappointments of patients.

Preparing patients prior to going in to see the GP,taking a brief history and basic readings in readiness for the GP appointment.

Dipping urine, taking blood pressure, ECGs &phlebotomy

Completing basic (non-opinion) forms and core elementsof some forms for the GP to approve and sign such as insurance forms, mortgage,benefits agency forms etc.

Explaining treatment procedures to patients

Helping the GP liaise with outside agencies ,getting an on-call doctor on the phone to ask advice or arrange admission whilethe GP can continue with their consultation(s)

Support the GP with immunisations/wound care.

Extracting information from clinical letters thatneeds coding, adding this to patient notes.

Supporting with QOF and IIF reviews.

Experience

Minimum of 1 year experience of working with healthcare professionals and or previous experience in the NHS or social care or relevant field Experience of working in a multi-disciplinary setting where influence and negotiation is required Experience of coordinating and liaising with multiple stakeholders or individuals to meet specified outcomes Polite and confident Flexible and cooperative Motivated Forward thinker High level of integrity and loyalty Sensitive and empathetic in distressing situations Ability to work under pressure.
