Job summary

Wenworth 1Primary Care Network (PCN) is looking to employ a Paramedic under theadditional roles reimbursement scheme (ARRS).The post holder will be accountable to the Primary Care Network, theindividual may also work across more than one GP Practice within the PCN insome cases where flexibility allows and for the purposes of support. We have anumber of roles available and would be happy to discuss this with you.Predominantly we are seeking individuals who can work hours, 30 hours, hours. Monday to Friday within core openting times.

Main duties of the job

Assessand triage patients, including same day triage, and as appropriate providedefinitive treatment (including prescribing medications following policy,patient group directives, NICE and local guidelines or make necessaryrecommendations to other members of the primary care team.

Useadvanced clinical skills to provide education to patients, promote self-careand encourage them to make informed choices about their treatment.

Performand interpret ECGs.

Workas part of an MDT within the PCN.

Undertakethe collection of pathological specimens including bloods, swabs and othersamples within scope of practice.

Maintainaccurate health records using Systm1, appropriate to the consultation, ensuringaccurate completion of necessary documentation associated with patient healthcare and registration with practice.

Communicateeffectively with patients and where appropriate family members and theircarers, where applicable, complex and sensitive information regarding theirphysical heath needs.

This includes the creation of additional roles in our practices including Clinical Pharmacists, Social Prescribers and Care Coordinators.

Job description

Job responsibilities

Triage, assess and treat patients at home, or inthe surgery when directed to do so. Make any necessary referrals incollaboration with the referring or duty clinician to other members of theprimary health care team, community or hospital teams or social care team asappropriate.

Assess, diagnose, plan, implement and evaluatetreatment, interventions and care for patients presenting with anundifferentiated diagnosis on an acute basis within the boundaries ofcompetence, seeking the advice of the referring or duty clinician if outsidelevel of knowledge and scope of practice.

Where appropriate, discuss with the referring orduty clinician doctor the needs of patients presenting with complex healthneeds or conditions requiring specific treatment, hospital admission orreferral.

Clinically examine and assess patient needs from aphysiological and psychological perspective, interpret test results and planclinical care accordingly.

Ensure clinical practice is safe and effective andremains within the boundaries of competence, and to acknowledge limitations.

Advise patients on general health care and minorailments, with referral to other members of the primary and secondary healthcare team as necessary.

Work as an autonomous practitioner within generalpractice or in the community setting, caring for patients and families, inaccordance with the Health and Care Professions Council Standards andProficiency, Standards of Conduct, Performance and Ethics. Ensure that personaland professional clinical standards and development are maintained.

Prescribe/issue medications as appropriatefollowing national policies, patient group directives and local pathways and inaccordance with own competence.

Formally and informally impart knowledge and skillsto colleagues promoting peer review and best practice within the workenvironment, ensuring the delivery of a safe and effective service.

Undertake the collection of pathological specimensincluding intravenous blood samples, swabs etc. Perform investigatoryprocedures requested by the GPs, only accepting to carry out procedures thatyou are appropriately trained to complete.

Qualifications

Essential

-Educated to a degree/diploma level in Paramedicine or equivalent experience. -Registered health care profession HCPC -Working towards developing masters level or equivalent capability in paramedic areas of practice. -prescribing qualification

Experience

Essential

Minimum of two years post qualification experience Three years experience as a band 6 or equivalent experience. Triage and patient examination skills. Knowledge of acute and long-term conditions. Prescribing qualification

Desirable

Worked within a Primary Care setting (GP Practice) Clinical System, System 1 / EMIS MSc Degree
