Job summary

We are looking for a pharmacy technicianto join our team at Wyvern Health PCN (Priory Road Medical Centre, The LawnMedical centre and Old Town Surgery) with a passion for primary care andworking with our patients and staff. You will join PCN pharmacy team ofpharmacists and pharmacy technicians who will provide support.

There will be a variety of roles whichwill bring interest and diversity to your day, such as medicine reconciliationand supporting patients post discharge from hospital, data collation formedicine optimisation, audits, policy development, day-to-day medication issueresolution, researching medication shortages, supporting patients in use ofmedicines & promoting electronic dispensing, managing safety alerts &staff training.

Technicians play an important role insupporting PCN repeat prescription coordinators, clinical pharmacists,community pharmacists and other members of the team to enhance patient safetyand improve patients health. The post holder will be a motivated individualwith an enthusiasm for people, quality improvement and care, with opportunitiesto enhance your clinical knowledge.

The role will encompass clinical,technical, educational, and administrative elements. There will be a mix offace to face, telephone, and video consultations with patients, providing avaried and interesting working day.

The Core activities of Pharmacy Technician

Daily prescription queries

Providingsupport to Clinical Pharmacists in managing daily patient prescriptionrequirements. To include the resolution of patient / pharmacy queries. Inparticular focussing on:Out ofstock medicationsPharmacyqueriesETPqueries

Discharge Summaries

Managingand processing discharge summaries and clinical letters.

Undertakingpatient facing and patient supporting roles to ensure effective medicines use,through shared decision-making conversations with patients.

Reviewing Patient Medication

RunningMedication Review clinics either patient facing or virtually, including in CareHomes.

Toinclude carrying out medicines optimisation tasks including effective medicineadministration ( checking inhaler technique), supporting medication reviewsand medicines reconciliation.

Whererequired, utilise consultation skills to work in partnership with patients toensure they use their medicines effectively.

Providingspecialist expertise, where competent, to address both the public health andsocial care needs of patients, including lifestyle advice, service information,and help in tackling local health inequalities.

Vaccination Programmes

Supportingvaccination programmes for patients and staff as required

MHRA Alerts

ManagingMHRA alerts and actions, including leading the receipt and understanding of thealert, communicating with PCN contacts and completing all associatedactions.

Actionsmay include, carrying out audits and patient searches, contacting patients andsending patient letters.

Eclipse Alerts

ManagingEclipse alerts. To include running searches and deciding on the appropriatecourse of action based on findings, communicating with PCN contacts andcompleting all associated actions.

Actionsmay include, carrying out audits and patient searches, contacting patients andsending patient letter.

Audits

Undertakingclinical audits, supporting quality improvement measures and contributing tothe Quality and Outcomes Framework and enhanced services.

Assistingin the delivery of medicines optimisation incentive schemes ( medicinesswitches) and patient safety audits.

Leadership

Beingthe point of contact for ICB for cost switches and cost savings

Supportinginitiatives for antimicrobial stewardship to reduce inappropriate antibioticprescribing.

Providingtraining and support on the legal, safe and secure handling of medicines,including the implementation of the Electronic Prescription Service(EPS).

Developingrelationships with other pharmacy technicians, pharmacists and members of themulti-disciplinary team to support integration of the pharmacy team acrosshealth and social care including primary care, community pharmacy, secondarycare and mental health.

Supervisingpractice reception / admin teams in sorting and streaming general prescriptionrequests, to allow GPs and clinical pharmacists to review the more clinicallycomplex requests.

Ensuringpractices security surrounding prescriptions pads, administration and stafftraining is robust.

Supportingthe implementation of national prescribing policies and guidance within GPpractices, care homes and other primary care settings

Taking acentral role in the clinical aspects of shared care protocols and liaising withspecialist pharmacists for more complex patients.

Supporttrainees such as Pre-registration Pharmacy technicians and FoundationPharmacists with their learning needs as required

Training requirements

All pharmacytechnicians must have completed or be enrolled in, be undertaking or beprepared to start the CPPE approved 15-month training pathway (PCPEP). Pharmacytechnicians must be registered with the General Pharmaceutical Council. Entryto the PCPEP programme will include the option for an accreditation of Prior Learning(APEL) process.

Experience of working as a qualified, registered pharmacy technician in primary care, community or hospital pharmacy
