Main duties of the job

The team also support paediatric palliation, within the community, and the successful candidate will be involved in working alongside our Palliative care team.

You will also be responsible for the updating of continuing care assessments, care plans, risk assessments, and providing education and support to both parents and carers, with management & supervision from senior nursing staff. Your role will also include some management input with our Band 3 & 4 care staff, that include Assistant Practitioners & Nursing Associates.

This post is an ideal opportunity to expand into the community setting, whilst maintaining strong links to the multidisciplinary team within the main hospital trust.
